慢性阻塞性肺疾病抗氧化剂最新药物研究进展

摘要 慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)是一种可以预防和治疗的常见病,以持续存在的呼吸道症状和气流受限为特征,由暴露于有害颗粒物或气体造成的气道和(或)肺泡异常所引起。呼吸困难、咳嗽和(或)咳痰是COPD的常见症状。COPD发病率和死亡率逐年上升,是全球第4大死亡原因。香烟烟雾产生的活性氧会损伤肺部结构及功能,引起肺部氧化应激失调,引发COPD。氧化应激是COPD发病机制中的关键,传统COPD治疗药物控制氧化应激的能力有限,且抗氧化应激临床证据不足,硫氧还蛋白和N-乙酰半胱氨酸作为机体自身抗氧化防御系统的分子,成为研发热点。该文对COPD氧化应激发病机制及抗氧化剂研究热点作一综述,旨在为临床提供参考。 Chronic obstructive pulmonary disease(COPD)is a common preventable and treatable disease characterized by persistent respiratory symptoms and airflow restriction,caused by airway and/or alveolar abnormalities resulting from significant exposure to harmful particulate matter or gases.Difficulty breathing,cough and/or phlegm are the most common symptoms of COPD.The incidence and mortality of COPD are increasing year by year and is the fourth leading cause of death worldwide.Reactive oxygen species produced by cigarette smoke can damage lung structure and function,causing pulmonary oxidative stress disorder and COPD.Oxidative stress is the key to the pathogenesis of COPD.Traditional COPD drugs have limited ability to control oxidative stress,and clinical evidence of antioxidant stress is insufficient.Thioredoxin and N-acetyl-L-cysteine as molecules of the body’s own antioxidant defense system have become research hotspots.This paper reviewed the pathogenesis of oxidative stress and research hotspot of antioxidants of COPD to provide reference for clinical practice.
