疏肝健脾法治疗肝郁脾虚型慢性咳嗽经验

Experience of Soothing the Liver and Strengthening the Spleen Method in the Treatment of Chronic Cough of Liver Depression and Spleen Deficiency Type

慢性咳嗽属于临床常见且诊治复杂的一种呼吸系统疾病。《黄帝内经》记载:“五脏六腑皆令人咳,非独肺也”,肝郁脾虚型慢性咳嗽的基本病因病机可概括为肝郁脾虚、痰气交阻、肺失宣降,故发为咳。以逍遥散为常用代表方的疏肝健脾法治疗此类咳嗽可获得较好的临床疗效,疏肝健脾法可使肝气得疏、脾气得健、痰气得消、肺气得降,轻者可单用疏肝健脾法,重者需加入理肺之品。此病证易生变证,临证时需谨而慎之,不拘于常,以助邪去正安。

Chronic cough is a kind of respiratory disease which is common in clinic and complicated in diagnosis and treatment.Huangdi Neijing records:“the five viscera and six viscera can lead to cough,not just the lungs”.The basic etiology and pathogenesis of Cchronic cough of liver depression and spleen deficiency type can be summarized as liver depression and spleen deficiency,phlegm and qi obstruction,and lung qi decline,so it is cough.The treatment of this type of cough has achieved good clinical efficacy by using Xiaoyao powder,a commonly used prescription for liver soothing and strengthening the spleen,that is,by soothing the liver and strengthening the spleen,the liver qi can be sparse,the spleen qi can be strengthened,the phlegm qi can be eliminated,and the lung qi can be reduced.Those with mild cough can use the method of soothing the liver and strengthening the spleen,but those who are seriously ill still need to add lung care products.Liver depression and spleen deficiency are prone to change,and it is necessary to be cautious and not confined to the norm when the syndrome is present,so as to remove pathogen.
