补阳还五汤联合低分子肝素钠预防膝关节置换术后下肢深静脉血栓形成临床观察

Effect of Buyang Huanwu Decoction Combined with Low Molecular Weight Heparin Sodium on Prevention of Deep Venous Thrombosis of Lower Extremity after Knee Arthroplasty

目的探讨补阳还五汤联合低分子肝素钠预防膝关节置换术后下肢深静脉血栓(DVT)形成的效果。方法选取2016年5月—2018年5月收治的膝关节置换手术患者76例,按照随机双盲法分为对照组和观察组,各38例。对照组给予低分子肝素钠治疗,观察组在对照组的基础上增加补阳还五汤治疗,观察并分析2组患者术前及术后凝血功能、血常规水平及术后DVT的发生率。结果观察组术后DVT发生率(2.63%)较对照组(21.05%)低,差异具有统计学意义(P<0.05);术后观察组血小板较对照组低,血红蛋白及红细胞较对照组高,差异具有统计学意义(P<0.05);术后观察组FIB及PT较对照组低,APTT较对照组高,差异具有统计学意义(P<0.05)。结论针对膝关节置换术手术患者,给予补阳还五汤联合低分子肝素钠可有效促进患者下肢静脉血液循环功能,改善其血液高凝状态,对患者术后下肢静脉血栓的形成起到重要作用。

Objective To investigate the effect of Buyang Huanwu decoction combined with low molecular weight heparin sodium on(DVT)formation of lower extremity deep venous thrombosis after knee arthroplasty.Methods From May 2016 to May 2018,76 patients undergoing knee arthroplasty were randomly divided into control group(n=38)and observation group(n=38).The control group was treated with low molecular weight heparin sodium,and the observation group was treated with Buyang Huanwu decoction on the basis of the control group.The coagulation function,blood routine level and the incidence of DVT after operation were observed and analyzed.Results The incidence of DVT after operation in the observation group(2.63%)was lower than that of the control group(21.05%),the platelet of the observation group was lower than that of the control group,the hemoglobin and red blood cells of the observation group were higher than those of the control group(P<0.05),the FIB and PT of the observation group were lower than those of the control group,and the APTT of the observation group was higher than that of the control group.The difference was statistically significant(P<0.05).Conclusion For patients undergoing knee arthroplasty,Buyang Huanwu decoction combined with low molecular weight heparin sodium can effectively promote the venous blood circulation function of lower extremity,improve the hypercoagulation state of blood,and play an important role in the formation of venous thrombosis of lower extremity after operation.
