加味苦参汤药浴治疗寻常型银屑病血热证临床观察

Clinical Observation on the Therapeutic Effect of Modified Kushen Decoction Bath in Treating Blood Heat Psoriasis Vulgaris

目的观察加味苦参汤药浴治疗寻常型银屑病血热证的临床疗效。方法将60例寻常型银屑病血热证住院患者按照随机数字表法分为治疗组和对照组,每组30例。2组患者均给予基础治疗:消银颗粒,口服,3.5 g/次,3次/d;复方甘草酸苷片,口服,3片/次,3次/d;每次药浴结束后于皮损部位外擦甘油。在基础治疗的同时,治疗组给予加味苦参汤药浴(30 min/次,1次/d),对照组给予臭氧水疗(30 min/次,1次/d)。2组患者于治疗第1周、治疗第2周比较银屑病面积和严重指数(PASI)评分,并于治疗第2周结束后进行临床疗效评价。结果治疗第1周、治疗第2周2组PASI评分较治疗前均明显下降(P<0.01);治疗第1周,治疗组PASI评分下降程度优于对照组(P<0.05);治疗第2周,治疗组PASI评分下降程度明显优于对照组(P<0.01)。治疗2周,治疗组总有效率为86.7%,高于对照组总有效率63.3%,差异有统计学意义(P<0.05)。结论加味苦参汤药浴治疗寻常型银屑病血热证具有较好疗效,不良反应少,值得临床参考应用。

Objective To observe the clinical effect of modified Kushen decoction bath in treating blood heat type psoriasis vulgaris.Methods According to the random number table,60 inpatients with blood heat psoriasis vulgaris were randomly divided into treatment group of 30 cases and control group of 30 cases.The patients in both groups were given basic treatment Xiaoyin granule,compound glycyrrhizin tablet,and external application of glycerol outside the skin lesions after each bath.At the same time of basic treatment,the treatment group was given modified Kushen decoction bath,and the control group was given ozone hydrotherapy.After treatment,PASI(psoriasis area and severity index)scores of the 1 st week and the 2 nd week were compared,and the clinical efficacy of both groups was observed after 2 weeks treatment.Results PASI scores of the two groups after one week and two-week treatment were significantly decreased as compared with those before treatment(P<0.01).In the first week of treatment,the PASI scores of the treatment group was better than that of the control group(P<0.05).At the 2 nd week of treatment,the decrease of PASI scores in the treatment group was significantly better than that in the control group(P<0.01).The total effective rate of the treatment group and control group after 2 weeks treatment was86.7%and 63.3%respectively,and there had statistical significance between groups(P<0.05).Conclusion Modified Kushen decoction bath has good curative effect and less adverse reactions in the treatment of blood heat type of psoriasis vulgaris,which is worthy of clinical reference.
