摘要
目的观察2.5%甘露醇等渗溶液联合多螺旋计算机断层扫描技术小肠造影(MSCTE)用于小肠疾病诊断的效能。方法回顾性分析167例疑似小肠病变患者,所有患者均接受MSCTE检查,并根据口服对比剂分为对照组77例和试验组90例。对照组予以口服纯水1500 m L;试验组予以口服2.5%甘露醇1500 m L。比较2组患者的十二指肠、空肠和回肠的充盈评分,并根据临床诊断结果分析2组的诊断效能。结果试验组和对照组的十二指肠充盈评分分别为3和2分,空肠充盈评分分别为2和2分,回肠充盈评分分别为3和2分,差异均有统计学意义(均P<0.05)。试验组和对照组的整体诊出率77.8%(70例/90例)和59.7%(46例/77例),差异有统计学意义(P<0.05)。试验组和对照组的腺癌诊断准确率分别为95.6%和87.0%,良性肿瘤诊断准确率分别为91.1%和68.8%,溃疡性肠膜炎诊断准确率分别为96.7%和81.8%,差异均有统计学意义(P<0.05,P<0.01)。结论口服2.5%甘露醇等渗溶液联合MSCTE可获得较好的肠道充盈效果,并且能显著提高小肠疾病的诊出率和准确率。
Objective nic solution combined with multislice spiral computed tomography enterography(MSCTE)in the diagnosis of small intestinal diseases.Methods A total of 167 patients with suspected small intestinal lesions were analyzed retrospectively.All patients were examined by MSCTE and were divided into control group(n=77 cases)and treatment group(n=90 cases)according to oral contrast medium.Control group was given water1500 m L.Treatment group received 2.5%mannitol isotonic solution1500 m L.The filling scores of duodenum,jejunum and ileum were compared between two groups,and the diagnostic efficacies of the two groups were analyzed according to the clinical diagnostic results.Results filling scores of duodenum in treatment and control groups were 3 and 2,the filling scores of jejunum were 2 and 2,the illing scores of ileum were3 and 2,the differences were statistically significant between two groups(all P<0.05).The total diagnosed rates in the treatment group and the control group were 77.8%(70 cases/90 cases)and 59.7%(46 cases/77 cases)with significance difference(P<0.05).The diagnostic accuracies of adenocarcinoma,benign tumor,ulcerative enteritis and ulcerative enteritis were 95.6%and 87.0%,91.1%and 68.8%,96.7%and 81.8%,respectively.The differences were statistically significant(P<0.05,P<0.01).Conclusion tion combined with MSCTE can obtain better intestinal filling effect,and can significantly improve the diagnosis rate and accuracy of small intestinal diseases.
