盐酸米诺环素软膏联合头孢拉定胶囊治疗牙周病患者的临床研究

Clinical trial of minocycline hydrochloride ointment combined with cefradine capsules in the treatment of periodontal disease

目的观察盐酸米诺环素软膏联合头孢拉定胶囊治疗牙周病的临床疗效及安全性。方法将100例老年牙周病患者随机分为对照组和试验组,每组50例。对照组给予头孢拉定胶囊每次0.5 g,tid,口服;试验组在对照组治疗的基础上,于患处或牙周袋内涂抹盐酸米诺环素软膏,qod。2组患者均治疗14 d。比较2组患者的临床疗效、龈沟液炎症因子水平的变化情况,以及药物不良反应的发生情况。结果治疗后,试验组和对照组的总有效率分别为96.00%(48例/50例)和78.00%(39例/50例),差异有统计学意义(P<0.05)。治疗后,试验和对照组的白细胞介素(IL)-1β分别为(233.68±56.05)和(378.78±96.89)pg·m L^-1,IL-8分别为(50.07±5.67)和(66.35±8.07)pg·m L^-1,IL-10分别为(16.52±4.44)和(11.93±2.69)pg·m L^-1,前列腺素E2分别为(70.90±9.79)和(83.65±11.43)pg·m L^-1,差异均有统计学意义(均P<0.05)。2组患者的药物不良反应均以胃肠反应和头晕为主。试验组和对照组的总药物不良反应发生率分别为10.00%和4.00%,差异无统计学意义(P>0.05)。结论盐酸米诺环素软膏联合头孢拉定胶囊治疗牙周病的临床疗效确切,其能有效地降低龈沟液IL-1β、IL-8和前列腺素E2水平,升高IL-10水平,且不增加药物不良反应的发生率。

Objective nocycline hydrochloride ointment combined with cefradine capsules in the treatment of periodontal disease.Methods tients with periodontal disease were randomly divided into control and treatment groups with 50 cases per group.Control group was given cefradine capsules 0.5 g per time,tid,orally.Treatment group was given minocycline hydrochloride ointment was applied to the affected area or periodontal pocket,qod,on the basis of control group.Two groups were treated for 14 days.The clinical efficacy,the changes of gingival crevicular fluid inflammatory factors and adverse drug reactions were compared between two groups.Results treatment and control groups were 96.00%(48 cases/50 cases)and78.00%(39 cases/50 cases)with significant difference(P<0.05).After treatment,the main indexes of treatment and control groups were compared:interleukin-1β(IL-1β)were(233.68±56.05)and(378.78±96.89)pg·ml^-1,IL-8 were(50.07±5.67)and(66.35±8.07)pg·ml^-1,IL-10 were(16.52±4.44)and(11.93±2.69)pg·ml^-1,prostaglandin E2 were(70.90±9.79)and(83.65±11.43)pg·ml^-1,the differences were statistically significant(all P<0.05).The adverse drug reactions of two groups were gastrointestinal reactions and dizziness.The total incidences of adverse drug reactions in the treatment and control groups were 10.00%and 4.00%without significant difference(P>0.05).Conclusion ointment combined with cefradine capsules has a definitive clinical efficacy in the treatment of periodontal disease,which can effectively reduce the levels of IL-1β,IL-8 and prostaglandin E2 in gingival crevicular fluid,increase the levels of IL-10,without increasing the incidence of adverse drug reactions.
