经阴道与腹腔镜行卵巢囊肿剥除术临床效果的Meta分析

Meta-analysis of the clinical effect of transvaginal and laparoscopic oophorocystectomy in patients

目的:对目前已发表的相关研究进行系统评价,以期评估经阴道与腹腔镜下卵巢囊肿剥除术的临床应用价值。方法:通过PubMed、知网和万方等检索资料,从建库到2022年,检索卵巢囊肿经阴道手术与腹腔镜手术进行对比的文献,筛选并收集相关文献资料,使用RevMan 5.4软件进行分析。结果:共选入22篇文献,研究总人数为2180例女性,其中接受经阴道手术治疗的女性有1074例,接受腹腔镜手术治疗的女性有1106例,经阴道组与腹腔镜组术中出血量、住院时间、术后并发症比较差异无统计学意义(P>0.05),经阴道组手术时间短于腹腔镜组(95%CI:-12.93~-1.65,P<0.05)、术后肛门首次排气时间早于腹腔镜组(95%CI:-6.79~-3.85,P<0.0001)。结论:两种术式相比,经阴道组在手术时间、术后肛门首次排气时间方面优于腹腔镜组。

Objective:The relevant studies published so far have been systematically reviewed to assess the clinical value of transvaginal and laparoscopic oophorocystectomy.Methods:Through the retrieval data of PubMed,CNKI and Wanfang,from the establishment of the database to 2022,the literatures of transvaginal surgery and laparoscopy of ovarian cysts were retrieved,and the relevant literatures were screened and collected.Then the RevMan 5.4 software was used for analysis.Results:A total of 22 literatures were selected,with a total of 2180 women involved in the studies,including 1074 women who underwent transvaginal surgery,1106 women who underwent laparoscopic surgery,and there was no statistical significance in the bleeding volume,total length of hospital stays and postoperative complications between the transvaginal group and the laparoscopic group(P>0.05);the duration of surgery was shorter in the transvaginal group(95%CI:-12.93~-1.65,P<0.05);and the first postoperative exhaust time was earlier than that in the laparoscopic group(95%CI:-6.79~-3.85,P<0.0001).Conclusion:Compared with the two surgical procedures,the transvaginal group was better than the laparoscopic group in terms of duration of surgery and time of first postoperative exhaustion.
