改良PLIF术治疗腰椎间盘突出症的效果观察

Effect of modified posterior lumbar interbody fusion(PLEF) with preservation of posterior ligament complex in the treatment of lumbar disc herniation

目的:探析保留后方韧带复合体(PLC)的改良后路腰椎椎间融合(PLIF)术治疗腰椎间盘突出症(LDH)的临床效果。方法:120例LDH患者,随机分为两组,每组60例。观察组实施保留PLC的改良PLIF术式,对照组实施传统PLIF术式;观察、分析两组的治疗效果。结果:经手术治疗后,观察组疼痛视觉模拟评分(VAS)、功能障碍指数(ODI)低于对照组(P<0.05);不良事件发生率23.3%,低于对照组55.0%(P<0.05);治疗有效率为96.7%,高于对照组78.3%(P<0.05)。结论:保留PLC的改良PLIF术治疗LDH,腰椎关节稳定性高,不良事件发生率低,治疗效果优于传统PLIF术。

Objective:To investigate the clinical effect of posterior lumbar interbody fusion(PLIF)with preservation of posterior ligament complex(PLC)in the treatment of lumbar disc herniation(LDH).Methods:120 patients with LDH were randomized into two groups,60 cases in each group.The observation group were given modified PLIF with PLC,while the control group were treated with traditional PLIF.The clinical effects of both groups were compared.Results:After surgical treatment,the VAS and ODI of the observation group were significantly lower than those of the control group(P<0.05);the incidence of adverse events in the observation group was 23.3%,significantly lower than 55.0%in the control group(P<0.05);the effective rate was 96.7%,which was higher than 78.3%in the control group(P<0.05).Conclusion:Modified PLIF with PLC in the treatment of intervertebral disc herniation hashigh lumbar joint stability and low incidence of adverse events,and the therapeutic effect is better than that of traditional PLIF.
