连续硬膜外麻醉无痛分娩对产妇产程、分娩结局及应激反应的影响

Effects of continuous epidural anesthesia for painless delivery on the labor process, labor outcome and stress response of puerperae

目的:探讨连续硬膜外麻醉无痛分娩对产程、分娩结局及应激反应的影响。方法:103例足月单胎初产妇,随机分为两组。对照组51例,采用常规方式分娩,观察组52例,采用连续硬膜外麻醉无痛分娩。观察、分析产程、妊娠结局和应激反应。结果:观察组产程时长较对照组短,自然分娩率高于对照组,剖宫产率低于对照组,差异有统计学意义(P<0.05);观察组血浆皮质醇、血糖、胰岛素水平低于对照组(P<0.05)。结论:连续硬膜外麻醉无痛分娩能够缩短产程,有助于产妇顺利娩出胎儿。

Objective:To investigate the effects of continuous epidural anesthesia for painless delivery on the labor process,labor outcome and stress response of puerperae.Methods:103 primiparae with term singleton fetus were randomized into 2 groups.51 cases in the control group received conventional delivery,while another 52 cases in the observation group received continuous epidural anesthesia for painless delivery.The labor process,labor outcome and stress response of the lying-in women in both groups were compared.Results:The duration of labor in the observation group was shorter than that in the control group,the rate of natural delivery was higher than that of the control group,and cesarean section rate was lower than that of the control group,which were of statistical differences(P<0.05);The plasma cortisol,blood glucose and insulin levels in the observation group were lower than those in the control group(P<0.05).Conclusion:Compared with conventional delivery,continuous epidural anesthesia for painless delivery can significantly shorten the duration of labor and facilitate the smooth delivery of the fetus.
