交锁髓内钉内固定治疗胫骨多段骨折的疗效及其对血清TGF-β1、FGF-2的影响

Effect of interlocking intramedullary nailing in the treatment of multi segmental tibial fractures and its influence on serum TGF-β1 and FGF-2

目的:研究交锁髓内钉内固定治疗胫骨多段骨折的疗效。方法:95例胫骨多段骨折患者按照随机数字表法分为两组,对照组47例予以微创经皮钢板内固定技术(MIPPO)治疗,研究组48例予以交锁髓内钉内固定治疗。观察两组手术效果、踝关节功能、骨再生因子水平、手术并发症。结果:两组手术时间无明显差异,研究组术中出血量低于对照组,骨折愈合时间短于对照组,差异有统计学意义(P<0.05)。研究组Kofoed评分、FGF-2、TGF-β1高于对照组,研究组并发症发生率为14.6%,低于对照组31.9%(P<0.05)。结论:交锁髓内钉内固定治疗胫骨多段骨折疗效好、踝关节功能恢复快。

Objective:To study the effect of interlocking intramedullary nail internal fixation in the treatment of multi segmental tibial fractures.Methods:95 cases of multi segmental tibial fractures were randomly divided into two groups.47 cases in the control group were treated with minimally invasive percutaneous plate fixation(MIPPO),and 48 cases in the study group were treated with interlocking intramedullary nail.The operation effect,ankle joint function,bone regeneration factor level and operation complications of the two groups were observed.Results:There was no significant difference in the operation time between the two groups.The intraoperative blood loss of the study group was lower than that of the control group,and the fracture healing time was shorter than that of the control group,the difference was statistically significant(P<0.05).The kofoed score,FGF-2 and TGF-β1 in the study group were higher than those in the control group.The incidence of complications in the study group was 14.6%,which was lower than that in the control group(31.9%)(P<0.05).Conclusion:Interlocking intramedullary nail internal fixation is effective in the treatment of tibial multi segment fracture and ankle joint function recovery is fast.
