超声引导下神经阻滞联合全身麻醉对下肢骨折手术患者麻醉效果的影响

Effects of ultrasound-guided nerve block combined with general anesthesia on patients with lower limb fracture surgery

目的:探讨超声引导下神经阻滞联合全身麻醉对下肢骨折手术患者麻醉效果的影响。方法:选择我院收治的80例下肢骨折手术患者作为研究对象,随机分为两组,每组40例。对照组行全身麻醉,观察组在此基础上麻醉诱导后,在超声引导下实施神经阻滞。比较两组不同时间段血流动力学指标、麻醉效果以及丙泊酚用量情况。结果:插喉罩及拔喉罩时,观察组收缩压、舒张压及心率均低于对照组,拔管、清醒及定向力恢复时间均早于对照组,且丙泊酚用量少于对照组,差异有统计学意义(P<0.05)。结论:超声引导下神经阻滞联合全身麻醉效果确切,可维持下肢骨折手术患者血流动力学稳定,同时减少麻醉药用量,促进患者早期苏醒及意识恢复。

Objective:To investigate the effects of ultrasound-guided nerve block combined with general anesthesia on patients with lower limb fracture surgery.Methods:80 patients with lower extremity fracture surgery admitted to our hospital were selected as the research subjects.They were randomized into 2 groups,with 40 cases in each.Patients in the control group underwent general anesthesia.Based on this,patients in the observation group received ultrasound-guided nerve block after induction of anesthesia.Hemodynamic indexes,anesthetic effects and propofol dosage at different time periods were compared between the two groups.Results:When the laryngeal mask was inserted and was removed,the systolic blood pressure,diastolic blood pressure and heart rate of the observation group were lower than that of the control group.The time of regaining consciousness,tube extraction and directional force were earlier than those in the control group,and the dosage of propofol were less than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Ultrasound-guided nerve block combined with general anesthesia has definite effects,which can maintain the hemodynamic stability of patients with lower extremity fracture surgery,reduce the dosage of anesthetics and promote the early recovery and regain consciousness of the patients at the same time.
