血清β-HCG、P水平联合检测对孕5~7周难免流产患者诊断效能的影响

Effects of the combined serum β-HCG and P level detection on the diagnostic efficiency of patients with inevitable abortion at 5 to 7 weeks of gestation

目的:探究孕5~7周难免流产患者采用血清人绒毛膜促性腺激素(β-HCG)、孕酮(P)水平联合检测对其诊断效能的影响。方法:选取我院诊治的孕5~7周难免流产患者58例(A组)、先兆流产患者45例(B组),同期健康体检孕妇40例(C组),均行血清β-HCG、P水平检测,比较3组血清β-HCG、P水平变化,并分析血清β-HCG、P水平单项检测及联合检测对难免流产诊断效能的影响。结果:A组血清β-HCG、P水平低于B组、C组(P<0.05),B组血清β-HCG、P水平低于C组(P<0.05);血清β-HCG水平单项检真阳性37例,血清P水平单项检出42例,联合检出55例;血清β-HCG、P水平联合检测孕5~7周难免流产患者的敏感度与准确度高于单项检测,且漏诊率低于单项检测(P<0.05),血清β-HCG、P水平单项检测特异度、误诊率与联合检测相比无明显差异(P>0.05)。结论:血清β-HCG、P水平在难免流产患者中呈低表达水平,二者联合诊断可显著提高孕5~7周难免流产患者诊断敏感度及准确度,有利于降低漏诊率。

Objective:To investigate the effects of the combined serum human chorionic gonadotropin(β-HCG)and progesterone(P)level detection on the diagnostic efficacy of inevitable abortion at 5 to 7 weeks of pregnancy.Methods:58 patients with inevitable abortion(Group A)and 45 patients with threatened abortion(Group B)treated in our hospital during 5-7 weeks of pregnancy,and 40 healthy controls(Group C)examined during the same period were selected.They were all performed the combined serumβ-HCG and P leveldetection.The changes ofβ-HCG and P level in 3 groups were compared,and the influence of single detection and combined detection on the diagnostic efficiency of inevitable abortion was analyzed.Results:β-HCG and P level in Group A were lower than those of Group B and C,and Group B were lower than Group C(P<0.05);37 cases were found positive in thesingle detection of serumβ-HCG level,42 cases were found positive in the single detection of P level,and 55 cases were found positive in the combined detection ofβ-HCG and P level;the sensitivity and accuracy of combined serumβ-HCG and P level detectionin patients with inevitable abortion at 5-7 weeks of pregnancy were higher than that of single detection,and the rate of missed diagnosis was lower than that of single detection(P<0.05),but there were no significant differences in the specificity and misdiagnosis rateof single detection ofβ-HCG and P level compared with the combined detection(P>0.05).Conclusion:Serumβ-HCG and P level is low in patients with inevitable abortion.The combined diagnosis of serumβ-HCG and P level can significantly improve the diagnostic sensitivity and accuracy of patients with inevitable abortion at 5-7 weeks of pregnancy,which is beneficial to reduce the rate of missed diagnosis.
