断指再植治疗手指末节完全离断伤临床分析

Clinical analysis of replantation of severed finger for the treatment of complete dislocation

目的:探讨断指再植对手指末节完全离断伤的临床治疗效果,并对断指再植成活率的影响因素进行分析。方法:回顾性分析我院收治的手指末节完全离断伤患者共计63例,其中采取断指再植治疗患者38例,采取非断指再植治疗患者25例,对比分析两组患者治疗效果,对影响断指再植成活率进行分析。结果:断指再植手术治疗与非断指再植相比,手术时间、愈合时间较短,术中出血量均较少,且术后并发症发生率低(P<0.05)。断指再植成活率的影响因素包括静脉桥接、缺血时间、受伤水平、待手术时间、年龄、血栓形成、血管痉挛。结论:断指再植手术是治疗手指末节完全离断伤较理想的治疗方法,需对影响该病断指再植成活率的影响因素予以重视,以进一步提高治疗效果。

Objective: To investigate the clinical treatment effects of replantation of severed finger on complete detachment of finger distal segment, and analyze the influencing factors of survival rate of severed finger replantation. Methods: A retrospective analysis of a total of 63 patients with complete severed distal segment of the finger treated in our hospital, of which 38 patients were treated with replanted fingers, and 25 patients with non-broken fingers were replanted. The efficacy and the survival rate of both groups were compared. Results: Compared with non-fingered finger replantation, surgical treatment of severed finger replantation has the advantages of shorter operation time and healing time, less intraoperative blood loss, and lower incidence of postoperative complications. The factors affecting the survival rate of severed finger replantation included venous bridging, ischemic time, injury level, waiting time for operation, age, thrombosis and vasospasm. Conclusion: Surgery for severed finger replantation is an ideal treatment method for complete detachment of finger distal segment. It is recessary to pay attention to the influencing factors that affect, the survival rate of the replanted finger to further improve the treatment effect.
