不同手术方式对分化型甲状腺癌治疗的效果及其影响因素分析

Effect of different surgical methods on differentiated thyroid cancer and its influencing factors

目的:探讨不同手术方式治疗分化型甲状腺癌的效果及影响疗效的因素。方法:104例分化型甲状腺癌患者随机分4组,每组26例。甲组行甲状腺全切术。乙组行甲状腺全切联合单双侧颈淋巴结清扫术。丙组行患侧腺叶切除联合峡部切除术。丁组行患侧腺叶切除联合峡部切除、对侧大部切除术。观察4组手术治疗效果,并分析影响预后的因素。结果:4组患者在治疗总有效率上差异无统计学意义(P>0.05)。分化型甲状腺癌预后与性别、年龄、TNM分期及病理类型相关(P<0.05)。结论:分化型甲状腺癌的手术治疗方式需根据肿瘤病理类型、分期、性别和年龄综合考虑,进行选择。

Objective: To explore the effect of different surgical methods on differentiated thyroid cancer and the factors influencing the effect. Methods: 104 patients with differentiated thyroid cancer were randomly divided into 4 groups, 26 cases in each group. Group A received total thyroidectomy. Group B underwent total thyroidectomy combined with unilateral and bilateral neck dissection. Group C received adenoidectomy and isthmectomy. In group D, adenoidectomy combined with isthmectomy and subtotal resection were performed. The effect of operation was observed in 4 groups, and the factors influencing the prognosis were analyzed. Results: There was no significant difference in the total effective rate between the 4 groups(P>0.05). The prognosis of differentiated thyroid cancer was correlated with sex, age, TNM stage and pathological type(P<0.05). Conclusion: The surgical treatment of differentiated thyroid cancer should be selected according to the pathological type, stage, sex and age.
