摘要
目的:探讨MRI平扫及T2* mapping在髌骨软化症的诊断价值。方法:经关节镜证实的病例组31个膝关节和正常对照组18个膝关节,分别行磁共振常规MRI平扫和T2*mapping检查。观察病例组及对照组的MRI平扫及T2* mapping伪彩图的色彩变化,并测量T2*值,应用独立样本t检验分析两者T2*值,应用χ^2检验分析两种方法是否存在差异。结果:病例组T2*值(29.0±4.6)ms,高于对照组的(24.9±2.2)ms,差异有统计学意义(P<0.05)。T2*mapping诊断的敏感度为90.3%,特异度为77.8%。MRI平扫的敏感度为58.1%,特异度为100.0%,两种诊断方法的差异有统计学意义(P<0.05),两者结合诊断的敏感度为90.3%,特异度为77.8%。结论:T2*mapping能早期定量评估髌骨软化症,且优于MRI平扫。
Objective: To investigate the value of MRI and T2* mapping in the diagnosis of patellar malacia. Methods: 31 knee joints in the case group and 18 knee joints in the normal control group were confirmed by arthroscopy. To observe the color change of MRI plain scan and T2* mapping pseudocolor image of case group and control group, and measure T2* value, analyze T2* value of the two methods with independent sample t test, and analyze whether there is difference between the two methods with χ^2 test. Results: The T2* value of the case group was(29.0±4.6)ms, higher than(24.9±2.2)ms of the control group, the difference was statistically significant(P<0.05). The sensitivity and specificity of T2* mapping were 90.3% and 77.8%, respectively. The sensitivity and specificity of plain MRI were 58.1% and 100.0% respectively. The difference between the two methods was statistically significant(P<0.05). The sensitivity and specificity of combined diagnosis were 90.3% and 77.8%, respectively. Conclusion: T2* mapping can evaluate patellar malacia quantitatively in early stage, and it is better than MRI.
