SP142-PD-L1在低分化胃腺癌中的表达及临床病理意义

摘要 目的分析SP142-PD-L1在低分化胃腺癌(PDGA)组织中的表达及与临床病理特征的相关性。方法采用SP142抗体,免疫组织化学方法检测163例低分化胃腺癌组织中PD-L1表达。结果 SP142-PD-L1在肿瘤细胞及间质免疫细胞表达,在非肿瘤性胃黏膜中不表达。肿瘤细胞及免疫细胞SP142-PD-L1表达率分别为36.81%、99.39%。肿瘤细胞SP142-PD-L1表达多见于肿瘤较大、实性型、高pTNM分期、存在脉管侵犯及肿瘤出芽的PDGA(P<0.001)。肿瘤细胞SP142-PD-L1高水平表达多见于实性型(P<0.01)及存在脉管侵犯的PDGA(P<0.05),免疫细胞SP142-PD-L1高水平表达多见于肿瘤较大、实性型、高pTNM分期、存在脉管侵犯及肿瘤出芽的PDGA(P<0.05,P<0.01)。肿瘤实质与周围间质交界区出现表达SP142-PD-L1免疫细胞者更易呈现肿瘤细胞PD-L1高表达(P<0.05)。结论 SP142-PD-L1在低分化胃腺癌中的表达具有独特特征,并且是潜在的预后标志物。肿瘤细胞或免疫细胞PD-L1高水平表达的肿瘤应被视为低分化胃腺癌特殊亚群。 Objective To investigate the feature of SP142-PD-L1 expression as well as the correlation between the expression of SP142-PD-L1 in poorly differentiated gastric adenocarcinoma(PDGA)and clinicopathological characteristics.Methods PD-L1 expressions on tumor cells and immune cells were evaluated with the use of Clone SP142 antibody by immunohistochemistry in 163 cases of PDGA. Results SP142-PD-L1 expression was observed in tumor and immune cells,but not in non-neoplastic gastric epithelium. SP142-PD-L1 expression on tumor and immune cells were identified in 36.81%and 99.39% cases respectively. SP142-PD-L1 expression on tumor cells was more frequently observed in PDGA with larger tumor size,solid-type histological features,high pTNM stage and with the presence of lymphovascular invasion as well as tumor budding(P<0.001). Highly expression of PD-L1 in tumor cells was more frequently detected in PDGA with solid-type histological features(P<0.01)and lymphovascular invasion(P<0.05). Highly expression of PD-L1 in immune cells was more frequently observed in cases with larger tumor size,solid-type histological features,high pTNM stage and with the presence of lymphovascular invasion as well as tumor budding(P<0.05,P<0.01). In addition,highly expression of PD-L1 in tumor cells was more often observed when immune cells within peritumoral area exhibiting SP142-PD-L1 expression(P<0.05). Conclusion The expression of SP142-PD-L1 in PDGA might harbor peculiar features and SP142-PD-L1 could be a potential prognostic biomarker. The tumor presented as highly expression of SP142-PD-L1 in either tumor or immune cells should be regarded as peculiar subgroups of PDGA.
