营养状态对结直肠癌患者术后手术部位感染的影响 被引量:1

Influence of Nutriture on Surgical Site Infection after Surgery for Colorectal Cancer Patients

摘要 目的:探讨营养状态对结直肠癌患者术后手术部位感染的影响。方法:选择2020年11月至2021年12月于我院接受手术治疗且营养状况正常的110例结直肠癌患者,以及接受同类手术治疗但营养状况异常(营养不良、超重或肥胖)的100例患者进行研究。比较不同营养状态组患者术后手术部位感染率,以及影响营养状态异常患者术后手术部位感染的危险因素。结果:营养正常组患者术后手术部位感染率为14.55%(16/110);营养状况异常组患者手术部位感染率为21.00%(21/100),其中营养不良组为16.00%(4/25),超重组为20.00%(13/65),肥胖组为40.00%(4/10)。营养状况异常组术后手术部位感染率明显高于营养正常组,P<0.05;营养异常患者中肥胖者手术部位感染率最高,P<0.05。营养异常者术后手术部位感染的危险因素为术前存在营养风险、Ⅲ类切口,保护性因素为腹腔镜辅助治疗,P<0.05。结论:结直肠癌伴营养状况异常者术后手术部位感染风险更高,尤其是肥胖者。术前应评估患者营养状况,积极予以干预,降低术后感染率。 Objective To investigate the influence of nutriture on postoperative surgical site infection in patients with colorectal cancer.Methods From November 2020to December 2021,110colorectal cancer patients with normal nutriture who received surgical treatment in authors'hospital and 100patients with abnormal nutriture(malnutrition,overweight or obesity)who received similar surgical treatment were selected into the study;then the postoperative surgical site infection rate of patients with above different nutriture,and the risk factors affecting postoperative surgical site infection of patients with abnormal nutriture were compared.Results The postoperative surgical site infection rate of patients with normal nutriture was 14.55%(16/110);The surgical site infection rate of patients with abnormal nutriture was 21.00%(21/100),including 16.00%(4/25)in malnutriture group,20.00%(13/65)in overweight group,and 40.00%(4/10)in obesity group.The postoperative surgical site infection rate in the group with abnormal nutriture was significantly higher than that in the group with normal nutriture(P<0.05);Obese patients had the highest surgical site infection rate among the patients with malnutriture(P<0.05).The risk factors of postoperative surgical site infection in patients with nutritional disorders were preoperative nutritional risk,ClassⅢincision,and the protective factors were laparoscopic adjuvant therapy(P<0.05).Conclusion Patients with colorectal cancer and abnormal nutriture have higher risk of postoperative surgical site infection,especially obese patients.The nutriture of patients should be evaluated before operation,and active intervention should be given to reduce the postoperative infection rate.
