克罗米芬联合r-hLH对卵巢功能减退患者卵巢功能、子宫动脉血流阻力及血清内分泌激素的影响

Effects of clomiphen combined with r-hLH on ovarian function, uterine arterial resistance and serum endocrine hormones in patients with hypoovarianism

目的:探究克罗米芬联合重组人促黄体激素(r-hLH)对卵巢功能减退患者卵巢功能、子宫动脉血流阻力及血清内分泌激素的影响。方法:按照随机数字表法将86例卵巢功能减退患者分为观察组和对照组,每组43例。对照组患者入组后于月经第3天开始服用克罗米芬,观察组在对照组基础上添加r-hLH治疗。测定患者治疗后窦状卵泡数(AFC)、卵巢体积(MOV)、卵巢间质动脉峰值流速(PSV)、阻力指数(RI)等卵巢功能指标及子宫动脉搏动指数(PI)、阻力指数(RI)、内膜厚度等子宫功能指标;测定患者血清中卵泡刺激素(FSH)、黄体生成激素(LH)及雌二醇(E2)水平。结果:观察组患者AFC数量、卵巢间质动脉PSV及子宫内膜厚度显著高于对照组,卵巢间质动脉RI值及子宫动脉血流参数PI、RI值均显著低于对照组;血清中E2、LH水平显著高于对照组;观察组患者妊娠率显著高于对照组患者,差异具统计学意义。结论:克罗米芬联合r-hLH可显著提高卵巢功能减退患者促排卵效果,改善体内激素水平,提高子宫内膜容受性。

Objective To explore the effects of clomiphene combined with recombinant human luteinizing hormone(r-hLH)on ovarian function,uterine arterial resistance,and serum endocrine hormones in patients with hypoovarianism.Methods 86 cases of patients with hypoovarianism were divided into observation group and control group with 43 cases in each group according to the random number table method.Control group was given orally clomiphene on the 3rd d of menstruation,and observation group was treated with r-hLH on the basis of control group.The ovary function indexes of antral follicle count(AFC),ovarian volume(MOV),peak systolic velocity of ovarian stroma(PSV)and resistance index(RI)and the uterine function indexes of uterine artery pulsatility index(PI),resistance index(RI)and intimal thickness were measured after treatment.The levels of serum follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)were measured.Results The AFC quantity,ovarian stroma PSV and endometrial thickness in observation group were significantly higher than those in control group,and the ovarian stromal artery RI value and uterine arterial blood flow indexes of PI and RI values were significantly lower than those in control group.The levels of serum E2 and LH were significantly higher than those in control group.The pregnancy rate in observation group was significantly higher than that in control group.Conclusions Clomiphene combined with r-hLH can significantly increase the ovulation induction,and improve the hormone level and the endometrial receptivity in patients with hypoovarianism.
