平喘固本汤联合水中太极治疗慢性阻塞性肺疾病肺肾气虚证的临床疗效

Clinical effect of Pingchuan Guben decoction combined with Ai-Chi in the treatment of chronic obstructive pulmonary disease with deficiency of lung and kidney qi and influence of cardiopulmonary function

目的:观察平喘固本汤联合水中太极治疗慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)肺肾气虚证的疗效及心肺功能的影响。方法:将80例COPD患者分为试验组和对照组,每组各40例。两组均予常规药物治疗、护理措施、指导功能锻炼和健康教育;对照组予水中太极训练,试验组在对照组的基础治疗上增加平喘固本汤治疗。分别在治疗前、治疗8周、结束3个月后进行评估。记录2组患者肺功能测定、心肺运动试验主要指标及圣乔治呼吸问卷(SGRQ)、慢性阻塞性肺疾病生活质量评分表(CAT)评分结果。结果:与治疗前比较,2组患者治疗8周、治疗结束3个月后第1秒用力呼气容积占预计值百分比(FEV1%pred)、第一秒用力呼气容积(FEV1)、1秒率(FEV1/FVC%)、第1秒用力呼气容积与肺活量的比值(FEV1/VC)、无氧阈占预计值百分比(AT/pred%)、最大摄氧(VO2max)、每公斤最大摄氧量(VO2max/kg)、代谢当量(METs)、峰值呼吸交换率(RER)、最大负荷(Wmax)等指标水平均有所提高,且与对照组治疗8周、治疗结束3个月后相比,试验组上述指标提高明显,差异具有统计学意义。本研究在临床治疗期间均未出现不良事件。结论:平喘固本汤联合水中太极训练可改善稳定期COPD患者的心肺功能、提升运动耐力,缓解临床症状,提高生活质量。

Objective To observe clinical effect of pingchuan guben decoction combined with Ai-Chi in the treatment of chronic obstructive pulmonary disease with deficiency of lung and kidney qi.Methods 80 cases were divided into experimental group and control group,40 cases in each group.Both groups were given routine drug therapy,nursing measures,guided functional exercise and health education.The control group received Ai-Chi training,and the experimental group received Ping Chuan Guben decoction in addition to the basic treatment of the control group.They were assessed before treatment,at 8 weeks,and 3 months after the end of treatment.Pulmonary function test,main indexes of cardiopulmonary exercise test,scores of St.George Respiratory Questionnaire(SGRQ)and chronic obstructive pulmonary disease Quality of Life Scale(CAT)were recorded.Results Compared with before treatment,FEV1%pred,FEV1,FEV1/FVC,FEV1/VC,AT%pred,VO2max,VO2max/kg,METs,peak RER,Wmax and other indexes in 2 groups were improved for 8 weeks and 3 months after treatment.Compared with the control group after 8 weeks of treatment and 3 months after the end of treatment,the above indexes in the experimental group were significantly improved,and the difference was statistically significant.In this study,there were no adverse events during clinical treatment.Conclusion Pingchuan guben decoction combined with Ai-Chi can improve cardiopulmonary function,exercise endurance,relieve clinical symptoms and improve quality of life in stable COPD patients.
