新活素对急性广泛前壁心肌梗死PCI术后心力衰竭患者短期预后的影响

Effect of rhBNP on Short-Term Prognosis of Heart Failure Patients with Acute Extensive Anterior Myocardial Infarction After PCI

目的观察新活素对急性广泛前壁心肌梗死经皮冠状动脉介入术(PCI)后心力衰竭患者心功能指标及短期预后的影响.方法选取2016年1月—2019年12月在中国医科大学附属第四医院就诊的急性广泛前壁心肌梗死PCI术后心力衰竭的患者58例,随机分为对照组和观察组.对照组给予常规抗心衰治疗,观察组在对照组基础上应用新活素72 h,观察对比两组心率、脑利钠肽(NT-proBNP)、周围静脉压、左室射血分数(LVEF)、左室舒张末内径(LVDD)、每搏量(SV)、住院时间及治疗总有效率.结果观察组患者临床治疗总有效率为96.6%,对照组为75.9%,观察组显著高于对照组(P<0.05).治疗后观察组患者血清NT-proBNP、心率、周围静脉压、LVDD、LVEF、SV明显优于对照组(P<0.05),且观察组住院时间缩短(P<0.05).结论新活素对急性广泛前壁心肌梗死PCI术后心力衰竭患者疗效确切,可以短期内改善预后及左室重构,缩短住院时间.

Objective To observe the effect of rhBNP on cardiac function and short-term prognosis of patients with heart failure after percutaneous coronary intervention(PCI)in patients with acute extensive anterior myocardial infarction.Methods From January 2016 to December 2019,58 patients with heart failure after PCI in the Fourth Affiliated Hospital of China Medical University were selected,they were randomly divided into control group and observation group.The control group was given conventional anti heart failure treatment,and the observation group was given rhBNP on the basis of the control group for 72 hours.The effective rate,heart rate,NT-proBNP,peripheral venous pressure,left ventricular ejection fraction(LVEF),left ventricular diastolic dimension(LVDD),stroke volume(SV)and hospitalization time were observed in the two groups.Results The total effective rate of the observation group was 96.6%,and that of the control group was 75.9%.The observation group was significantly higher than the control group(P<0.05).After treatment,serum NT-proBNP,heart rate,peripheral venous pressure,LVDD,LVEF,SV of the observation group were significantly better than those of the control group(P<0.05),and the hospitalization time of the observation group was shortened(P<0.05).Conclusion RhBNP can improve the prognosis and left ventricular remodeling in a short time and shorten the hospitalization time.
