小儿尿道下裂患者包皮组织中COX-2、TIMP1表达及临床意义

摘要 目的检测环氧合酶-2(COX-2)、金属蛋白酶组织抑制因子1(TIMP1)在小儿尿道下裂患者包皮组织中的表达情况,并探讨其与小儿尿道下裂之间的关系.方法选取2020年6月至2021年6月在我院泌尿外科收治的小儿尿道下裂患者151例为尿道下裂组,根据阴茎伸直后尿道外口位置分为轻型组(48例)、中型组(50例)、重型组(53例),另选取71例尿道外口正常者为正常对照组;通过免疫组化法(SP)检测所有受试者包皮组织中COX-2、TIMP1的表达情况,通过Spearman法分析小儿尿道下裂患者包皮组织中COX-2、TIMP1之间的相关性;Logistic回归分析影响小儿尿道下裂的有关因素.结果与对照组比较,小儿患者COX-2阳性表达率升高,TIMP1阳性表达率降低,与轻型组比较,中型组、重型组小儿尿道下裂患者COX-2阳性表达率升高,TIMP1阳性表达率降低(P<0.05);与中型组比较,重型组小儿尿道下裂患者COX-2阳性表达率升高,TIMP1阳性表达率降低(P<0.05);经Spearman法分析小儿尿道下裂患者COX-2、TIMP1水平呈负相关(P<0.05);多因素Logistic回归分析结果表明COX-2、TIMP1是小儿尿道下裂患者发生的影响因素.结论小儿尿道下裂患者COX-2阳性表达、TIMP1阴性表达可能与尿道下裂的发生有关. Objective To detect the expression of cyclooxygenase-2(COX-2)and tissue inhibitor of metalloproteinase 1(TIMP1)in the foreskin tissue of children with hypospadias,and to explore the relationship between them and children with hypospadias.Methods A total of 151 pediatric hypospadias patients who were admitted to the Department of Urology in our hospital from June 2020 to June 2021 were selected as the hypospadias group.According to the position of the external urethra after penis extension,they were divided into mild group(48 cases),moderate group(50 cases),severe group(53 cases),in addition,71 cases of normal external urethral orifice were selected as normal control group;The expressions of COX-2 and TIMP1 in prepuce of all subjects were detected by immunohistochemistry(SP),and the correlation between COX-2 and TIMP1 in prepuce of children with hypospadias was analyzed by Spearman’s method;the related factors of affecting hypospadias in children were detected by Logistic regression.Results The positive expression rate of COX-2 was higher in pediatric patients than those in control group,on the contrary,the positive expression rate of TIMP1 was lower in pediatric patients than those in control group,the positive expression rate of COX-2 in children with hypospadias were higher in the moderate and severe groups than those in mild group,while the positive expression rate of TIMP1 were lower in the moderate and severe groups than those in mild group(P<0.05);the positive expression rate of COX-2 was higher in severe group than those in moderate group,while the positive expression rate of TIMP1 were lower in severe groups than those in moderate group(P<0.05);The results of correlation analysis showed that the levels of COX-2 in children with hypospadias were negatively correlated toTIMP1’s by Spearman’s method(P<0.05);multivariate Logistic regression analysis showed that COX-2and TIMP1 were the influencing factors of hypospadias in children.Conclusion The positive expression of COX-2 and negative expression of TIMP1 in children with hypospadias may be related to the occurrence of hypospadias.
