全身麻醉复合硬膜外麻醉在老年腹部手术患者的应用效果

Effect of General Anesthesia Combined with Epidural Anesthesia in Elderly Patients Undergoing Abdominal Surgery

目的研究全身麻醉复合硬膜外麻醉在老年腹部手术患者的应用效果.方法选取2019年11月-2021年1月期间在我院行腹部手术的84例老年患者为研究对象,通过抽签法的方式将其分为了两组,分别为对照组(n=42)和观察组(n=42),对照组使用单纯全身麻醉,观察组则是使用全身麻醉复合硬膜外麻醉,观察对比两组患者的麻醉效果临床指标、麻醉药物用量、血流动力学指标.结果观察组患者自主呼吸时间、拔管时间、完全清醒时间均短于对照组(P<0.05);观察组患者所使用的的四种维持麻醉药的剂量明显小于对照组(P<0.05);两组患者心率、平均动脉压(P>0.05),切皮时、术中30min以及术毕心率、平均动脉压均有不同程度变化,但是观察组均小于对照组(P<0.05).结论对于行腹部手术的老年患者来说,采用全身麻醉复合硬膜外麻醉对血流动力学影响小,麻醉起效快速,且麻醉药物用量少,具有相对更优的应用优势.

Objective To study the effect of general anesthesia combined with epidural anesthesia in elderly patients undergoing abdominal surgery.Methods 84 elderly patients who underwent abdominal surgery in our hospital between November 2019 and January 2019 were 2021 into two groups by drawing lots,the patients were divided into control group(n=42)and observation group(n=42).The patients in the control group were given general anesthesia,while the patients in the observation group were given general anesthesia combined with epidural anesthesia,the clinical indexes of anesthetic effect,the dosage of anesthetic drugs and the indexes of Hemodynamics were compared between the two groups.Results The time of spontaneous respiration,extubation and time of full consciousness in the observation group were shorter than those in the control group(P<0.05),the dose of four kinds of maintenance anesthetics used in the observation group were significantly lower than those in the control group(P<0.05),the heart rate and mean arterial pressure in the two groups(P>0.05),the heart rate and mean arterial pressure at the time of skin incision,30 min during operation and at the end of operation in the Observation Group were changed in different degrees,but all in the observation group were lower than those in the control group(P<0.05).Conclusions General anesthesia combined with epidural anesthesia is a less Hemodynamics,more effective and less intensive method for elderly patients undergoing abdominal surgery.
