老年维持性血液透析患者蛋白质能量消耗情况及危险因素

Risk Factors and Protein Energy Wasting in Elderly Patients with Maintenance Hemodialysis

摘要 目的分析老年维持性血液透析患者蛋白质能量消耗(PEW)情况及危险因素.方法选取2019年1月至2021年1月在我院进行维持性血液透析的62例老年患者,根据PEW诊断标准将其分为非PEW组(n=25)和PEW组(n=37).对两组患者年龄、性别、体重指数、蛋白质摄入、透析时间、C反应蛋白、糖尿病发生率等资料进行单因素比较,选取存在显著差异的指标行非条件Logistic多元逐步回归分析,分析影响患者出现PEW的危险因素.结果两组性别、体重指数比较无统计学差异(P>0.05);经非条件Logistic多元逐步回归分析可知,年龄、蛋白质摄入、透析时间、C反应蛋白、糖尿病发生率均是影响患者PEW发生的危险因素(P<0.05).结论年龄、蛋白质摄入、透析时间、C反应蛋白、糖尿病发生率均会对老年维持性血液透析患者蛋白质能量消耗造成影响,应尽量采取干预措施改善影响因素. Objective This paper aims to analyze the protein energy wasting(PEW)and risk factors of elderly patients with maintenance hemodialysis.Methods A retrospective analysis was performed on the clinical data of 62 elderly patients undergoing maintenance hemodialysis in the hospital between January 2019 and January 2021,and the patients were divided into non-PEW group(n=25)and PEW group(n=37)according to the PEW diagnostic criteria.Univariate comparison was performed on data such as age,gender,body mass index,protein intake,dialysis time,C-reactive protein and incidence rate of diabetes mellitus,and the indicators with significant differences were selected for non-conditional Logistic multivariate stepwise regression analysis,and the risk factors for PEW were analyzed among the patients.Results There were no statistical differences between the two groups in terms of gender and body mass index(P>0.05).Non-conditional Logistic multivariate stepwise regression analysis showed that age,protein intake,dialysis time,C-reactive protein and incidence rate of diabetes mellitus were the risk factors affecting the occurrence of PEW in patients(P<0.05).Conclusion Age,protein intake,dialysis time,C-reactive protein and incidence rate of diabetes mellitus all affect the protein energy wasting of elderly patients with maintenance hemodialysis,and it is necessary to take intervention measures as far as possible to improve the influencing factors.
