尿毒症患者透析早期应对方式及影响因素分析

Analysis of Early Coping Style of Dialysis in Patients with Uremia and Its Influencing Factors

目的探讨尿毒症患者透析早期应对方式及影响因素.方法回顾性分析2015年1月至2019年7月在我院进行透析治疗的尿毒症患者80例临床资料,使用医学应对量表进行调查,依据最终应对方式的不同分组,将采取回避、屈从应对方式的30例患者临床资料归为对照组,将采取面对应对方式的50例患者临床资料归为观察组,记录并分析其可能引发尿毒症患者透析早期应对方式改变的相关危险因素.结果两组性别、居住情况、年龄、照顾者类型、婚姻状况、居住地、透析方式比较,差异无统计学意义(P>0.05);观察组家庭人均月收入水平(<4000元)、年龄、文化程度(高中及以下)、合并疾病数(≥2种)、社会支持(低等水平)、有并发症占比均高于对照组,差异有统计学意义(P<0.05);经Logistic多因素分析显示,家庭人均月收入水平(<4000元)、年龄、文化程度(高中及以下)、合并慢性疾病数(≥2种)、社会支持(低等水平)、有并发症是影响尿毒症患者透析早期应对方式的危险因素(OR>1,P<0.05).结论家庭人均月收入水平低、文化程度低、慢性疾病数多、社会支持为低等水平、有并发症是造成尿毒症患者透析早期应对方式改变的影响因素.

Objective This paper aims to investigate the early coping style of dialysis in patients with uremia and its infu-encing factors.Methods The clinical data of 80 patients with uremia who underwent dialysis in the hospital from January 2015 to July 2019 were retrospectively analyzed.The medical response scale was used for investigation.The patients were divided into groups according to the final coping style,the elinical data of 30 patients who adopted the avoidance and submission were casi-fied as the control group,and the clinical data of 50 patients who adopted the coping style were classified as the observation group.The related risk factors that may trigger changes in the early coping style of dialysis in patients with uremia were recorded and analyzed.Results There was no statistical difference in the gender,residence,age,type of caregiver,marital status,place of residence,and dialysis method between the two groups(P>0.05);The proportion of family monthly income per capita(<4000 yuan),age,education level(high school and below),number of combined discase(≥2 types),social support(low level),and complications in the observation group was higher than that in the control group(P<0.05);Logistic multi-fac-tor analysis showed that family monthly income per capita(<4000 yuan),age,education level(high school and below),num-ber of combined disease(≥2 types),social support(low level),and complications were the risk factors for early coping style of dialysis in patients with uremia(0R>1,P<0.05).Conclusion L ow family monthly income per capita,low education level,multiple chronic diseases,low level of social support,and complications are the infuencing factors for the changes in the early coping style of dialysis in patients with uremia.
