头皮小汗腺汗孔癌一例

A case of eccrine porocarcinoma on the scalp

33岁男性患者,头皮肿物6年余,肿物切除术后多次复发伴破溃、渗出。皮肤科查体:患者头顶部可见一直径约15 cm的皮肤肿物。皮损组织病理检查:真皮内肿瘤细胞团块,肿瘤团块大小不一,瘤体由基底样细胞组成,部分区域可见管腔分化,部分细胞胞质空,核异形性明显。免疫组化染色:EMA阳性,CEA在导管分化部分呈阳性表达。PAS染色阳性。诊断:小汗腺汗孔癌。

A 33-year-old male patient presented with a mass on the scalp for more than 6 years.After resection,the tumor recurred several times,accompanied by ulceration and exudation.Dermatological examination showed a mass about 15 cm in diameter on the scalp.Histopathology revealed tumor masses in different sizes in the dermis.The tumor was composed of basaloid cells.Noticeable luminal differentiation,nuclear atypia and cytoplasmic vacuoles were also seen.Immunohistochemical results showed that EMA was positive,and CEA was positive in the differentiation part of the catheter.Additionally,PAS was positive.The case was diagnosed as eccrine porocarcinoma.
