摘要
目的:比较经下肢、经腹部皮下通路腹腔镜腹沟股淋巴清扫术(VEIL)在外阴癌手术患者中的应用效果。方法收集2011年6月-2019年8月医院52例外阴癌手术患者临床资料,根据不同通路路径分为经腹部VEIL组28例、经下肢VEIL组24例。经腹部VEIL组采用经腹部皮下通路VEIL,经下肢VEIL组采用经下肢皮下通路VEIL,比较两组手术相关指标、并发症、复发率。结果两组手术时间、出血量、淋巴结清扫数、住院时间比较,差异无统计学意义(P>0.05),经下肢VEIL组引流时间、引流量短于或少于对照组(P<0.05)。经腹部VEIL组与经下肢VEIL组并发症比较(14.29%vs 8.33%),差异无统计学意义(x^(2)=0.449,P>0.05)。随访1年,经腹部VEIL组与经下肢VEIL组复发率比较(7.14%vs 4.17),差异无统计学意义(x^(2)=0.211,P>0.05)。结论经腹部VEIL与经下肢VEIL均是治疗外阴癌的有效方法,经下肢VEIL能够缩短引流时间,减少引流量。
Objective:To compared efficacy of vdieo endoscopic inguinal lymphadenectomy(VEIL)via limb/hypogastric subcutaneouc approach for patient with vulvar cancer.Methods:52 patients with vulvar cancer from June 2011 to August 2019 were divided into via hypogastric VEIL group(28 cases)via limb VEIL group(24 cases).Via hypogastric VEIL group used via hypogastric subcutaneouc approach VEIL,Via limb VEIL group used via limb subcutaneouc approach VEIL.The surgical indicators,complications and recurrence rates were compared between two groups.Result:There were no statistical significance of operation time,blood loss,lymph node clearance and hospital stay between the two groups(P>0.05),Via limb VEIL group drainage time and the drainage volume shorter or less than Via hypogastric VEIL group(P<0.05).There was no statistical significance of complications between Via limb VEIL group and Via hypogastric VEIL group(14.29%vs 8.33%)(x^(2)=0.449,P>0.05).Followe-up for 1year,There was no statistical significance of recurrence rates between Via limb VEIL group and Via hypogastric VEIL group(7.14%vs 4.17)(x^(2)=0.211,P>0.05).Conclusion:Via limb VEIL and Via hypogastric VEIL are effective in the treatment of Vulvar cancer.Via limb VEIL can shorten the drainage time and reduce the amount of drainage.
