卡前列素氨丁三醇、麦角新碱在高龄产妇前置胎盘剖宫产术中的应用分析

Application Analysis of Carboprost Tromethamine and Ergometrine in Cesarean Section of Placenta Previa in Elderly Puerperae

目的:分析卡前列素氨丁三醇、麦角新碱在高龄产妇前置胎盘(PP)剖宫产术中的应用效果.方法:选取2019 年 3 月至 2022 年 3 月本院确诊为PP并行剖宫产术分娩的高龄产妇 117 例为研究对象,根据治疗方案不同分为A组(n =55)和B组(n =62).A组给予卡前列素氨丁三醇治疗,B组采用麦角新碱治疗.观察并比较两组术中及术后 3h、1d出血量、术前和术后血红蛋白含量与红细胞计数和术后各项指标.结果:两组产妇术中、术后 3h、术后 1d出血量,术前和术后的血红蛋白含量、红细胞计数,术后各项指标比较,差异均无统计学意义(P>0.05),唯B组治疗费用低于A组(P<0.05).结论:卡前列素氨丁三醇和麦角新碱在高龄产妇PP剖宫产术中应用效果较接近,麦角新碱费用相对更低,能减轻产妇经济负担.故推荐临床预防剖宫术中、术后出血时首选麦角新碱.

Objective:To analyze the application effects of carboprost tromethamine and ergometrine in cesarean section for placenta previa(PP)in elderly puerperae.Methods:One hundred and seventeen elderly puerperae who were diagnosed with PP and underwent cesarean section delivery in our hospital from March 2019 to March 2022 were selected into this study,and according to different treatment plans,they were divided into the group A(n=55)and the group B(n=62).The group A was treated with carboprost tromethamine intrauterine hemostasis program,and the group B was treated with ergometrine intrauterine hemostasis program.The hemorrhage during and after operation,the hemoglobin and red blood cell counts before and after surgery,the various postoperative indexes were observed and compared between the two groups.Results:There were no significant differences between the two groups in the blood loss during operation and 3 hours and one day after surgery,and in the hemoglobin,red blood cell count before and after surgery and other postoperative indexes(P>0.05).And the treatment cost of group B was lower than that of group A(P<0.05).Conclusion:The therapeutic effect of carboprost tromethamine and ergometrine in the treatment of bleeding during cesarean section of PP in elderly puerperae is similar,the cost of ergometrine is relatively lower,which can reduce the economic burden of puerperae,so it is recommended to use ergometrine as the first choice for clinical prevention of bleeding during and after cesarean section.
