摘要
目的:观察乙型肝炎肝硬化(HBV-LC)患者急性上消化道出血(UGB)发生情况并分析其影响因素.方法:选取 2019 年 1 月至 2021 年 1 月本院收治的 92 例HBV-LC患者为研究对象.将其中发生急性UGB患者 28例纳入发生组;将未发生急性UGB的患者 64 例纳入未发生组.比较两组患者临床资料,分析影响HBV-LC急性UGB发生的相关因素.结果:两组患者食管静脉曲张程度、腹水量、胃左静脉内径、白蛋白水平比较,差异均有统计学意义(P<0.05);经多因素 Logistic 回归分析结果显示,重度食管静脉曲张、胃左静脉内径大、腹水量高是HBV-LC患者发生急性 UGB 的危险因素(OR>1,P<0.05);白蛋白水平高是保护因素(OR<1,P<0.05).结论:重度食管静脉曲张、胃左静脉内径大、腹水量高是HBV-LC患者发生急性UGB的危险因素,白蛋白水平高是保护因素.
Objective:To observe the occurrence of acute upper gastrointestinal bleeding(UGB)in patients with hepatitis B virus liver cirrhosis(HBV-LC)and to analyze its influencing factors.Methods:A total of 92 patients with HBV-LC who were treated in our hospital from January 2019 to January 2021 were selected as the research subjects.Among them,28 patients with acute UGB were included in the occurrence group,64 patients without acute UGB were included in the non-occurrence group.The clinical data of patients in both groups were compared,and the related factors affecting the occurrence of acute UGB in patients with HBV-LC were analyzed.Results:There were statistically significant differences between the two groups in the degree of esophageal varices,ascites volume,diameter of left gastric vein and albumin level(P<0.05).Through multiple Logistic regression analysis,the results showed that severe esophageal varices,large diameter of left gastric vein and high ascites were risk factors for acute UGB in patients with HBV-LC(OR>1,P<0.05),and high albumin level was the protective factor(OR<1,P<0.05).Conclusion:Severe esophageal varices,large diameter of left gastric vein and high volume of ascites are risk factors for acute UGB in patients with HBV-LC,high albumin levels are the protective factors.
