全膝关节置换术后恐动症发生的影响因素分析

Analysis of Influencing Factors of Phobia after Total Knee Arthroplasty

目的:探讨全膝关节置换术(TKR)后恐动症发生的影响因素.方法:选取 2019 年 1 月至 2020 年 8 月在邓州市人民医院行TKR且术后发生恐动症的 60 例患者,纳入观察组;另选取同期行TKR且未发生恐动症的 60 例患者,纳入对照组.比较两组患者一般临床资料、疼痛、焦虑抑郁、膝关节功能情况,分析行TKR患者术后恐动症发生的影响因素.结果:观察组疼痛视觉模拟评分(VAS)、医院焦虑抑郁量表(HADS)评分高于对照组,膝关节功能评分表(HSS)评分低于对照组(P<0.05);经多因素非条件Logistic回归分析结果显示,VAS评分及HADS评分高、HSS评分低均是行TKR后发生恐动症的影响因素(P<0.05).结论:术后疼痛、焦虑抑郁情绪强、膝关节功能差是TKR患者术后发生恐动症的影响因素.

Objective:To explore the influencing factors of phobia after total knee arthroplasty(TKR).Methods:Sixty patients who underwent TKR in Dengzhou People′s Hospital from January 2019 to August 2020 and developed postoperative phobia were included in the observation group.Meanwhile,another 60 patients who underwent TKR during the same period and did not develop phobia were included in the control group.The general information,pain,anxiety,depression and knee function of two groups were compared,and the influencing factors of postoperative phobia of TKR patients were analyzed.Results:The scores of visual analogue scale(VAS)and hospital anxiety and depression scale(HADS)in the observation group were higher than those in the control group,and the scores of knee function scale(HSS)were lower than those in the control group(P<0.05).Univariate and multivariate analysis showed that high VAS score,high HADS score and low HSS score were the influencing factors of phobia after TKR(P<0.05).Conclusion:Postoperative pain,strong anxiety and depression,and poor knee function are the influencing factors of postoperative phobia in TKR patients.
