糖尿病肾病患者发生高尿酸血症的影响因素分析

Analysis on Factors of Hyperuricemia in Patients with Diabetic Nephropathy

目的:分析糖尿病肾病(DN)患者发生高尿酸血症(HUA)的影响因素.方法:回顾性分析,收集2020年1月至2021年1月灵宝市第一人民医院接收的发生HUA的66例DN患者资料,纳入发生组,另收集同期未发生HUA的66例DN患者资料,纳入未发生组.查阅患者临床资料并记录研究所需资料,将可能的影响因素纳入,经单因素、多因素分析找出DN患者发生HUA的影响因素.结果:发生组患者的文化程度、体重指数(BMI)、DN病程、停用降尿酸药物占比与未发生组患者相比,差异有统计学意义(P<0.05);组间年龄、性别比较,差异无统计学意义(P>0.05).经二分类Logistic回归分析结果显示,文化程度初中及以下、BMI过高、DN病程长、停用降尿酸药物是DN患者发生HUA的危险因素(OR>1,P<0.05).结论:DN患者发生HUA可能与患者文化程度低、BMI过高、DN病程长、停用降尿酸药物等因素有关,可据此提出干预方法来降低HUA的发生率.

Objective:To analyze the influencing factors of hyperuricemia(HUA)in patients with diabetic nephropathy(DN).Methods:A retrospective analysis was conducted.The data of 66 DN patients with HUA received by Lingbao First People's Hospital from January 2020 to January 2021 were collected as the occurrence group,while the data of 66 DN patients without HUA in the same period were collected as the nonoccurrence group.Reviewed the clinical data of patients and recorded the data required for the study,then included possible influencing factors.The influencing factors of HUA in patients with DN were found by univariate and multivariate analysis.Results:The educational level,body mass index(BMI),course of DN and the proportion of discontinuing uric acid lowering drugs in the occurrence group were significantly different from those in the non occurrence group(P<0.05);There was no statistically significant difference in age and sex between the groups(P>0.05).After binary Logistic regression analysis,the results showed that education level of junior middle school or below,high BMI,long course of DN and discontinuation of uric acid lowering drugs were the risk factors of HUA in patients with DN(OR>1,P<0.05).Conclusion:HUA in patients with DN may be related to low educational level,high BMI,long course of DN and discontinuation of uric acid lowering drugs,intervention methods can be proposed to reduce the incidence of HUA.
