急性脑梗死经介入取栓后出血性转化的Logistic回归分析

Logistic Regression Analysis of Hemorrhagic Transformation after Interventional Thrombectomy in Acute Cerebral Infarction

目的:急性脑梗死经介入取栓后出血性转化的Logistic回归分析.方法:选取2018年1月至2020年3月期间于本院就诊的106例急性脑梗死(ACI)患者的临床资料为研究对象,根据取栓治疗后患者是否出血分为出血组(n=28)及未出血组(n=78),同时调查取栓出血性转化的影响因素并进行分析.结果:106例ACI患者经介入取栓后,有28例患者出现出血性转化,出血性转化发生率为26.42%.年龄、性别、血糖等为非影响患者取栓后出血性转化的单因素(P>0.05);NIHSS评分、舒张压(>90mmHg)(1mmHg=0.133KPa)、收缩压(>140mmHg)、心房颤动为影响取栓治疗后出血性转化的单因素(P<0.05).经多因素logistic回归分析得:舒张压、收缩压、NIHSS评分、心房颤动是影响患者经介入取栓后出血性转化的独立危险因素(P<0.05).结论:在ACI患者经介入取栓后应密切注意患者舒张压、收缩压、心房颤动、NIHSS评分变化,为临床预防患者出现脑出血转化提供有效的参考依据.

Objective:Logistic regression analysis of hemorrhagic transformation after interventional thrombectomy in acute cerebral infarction(ACI).Methods:The clinical data of 106 patients with ACI admitted to our hospital from January 2018 to March 2020 were selected as the subjects.The patients were divided into a bleeding group and a non-bleeding group according to whether they were bleeding after thrombectomy,and the influencing factors of thrombectomy hemorrhagic transformation were investigated and analyzed.Results:After interventional thrombectomy in 106 parents with ACI,28 patients showed hemorrhagic transformation,and the incidence of hemorrhagic transformation was 26.42%.Age,gender,and blood glucose were the single factors that did not affect the hemorrhagic transformation after thrombectomy(P>0.05).The National Institutes of Health stroke scale(NIHSS)score,diastolic blood pressure(>90mmHg)(1mmHg=0.133KPa),systolic blood pressure(>140mmHg)and atrial fibrillation were the single factors affecting the hemorrhagic transformation after thrombolysis(P<0.05).Multivariate logistic regression analysis showed that diastolic blood pressure,systolic blood pressure,NIHSS score,and atrial fibrillation were independent risk factors affecting the hemorrhagic T—transformation after interventional thrombolysis(P<0.05).Conclusion:The changes in diastolic blood pressure,systolic blood pressure,atrial fibrillation,and NIHSS score should be paid close attention to in ACI patients after interventional thrombolysis,providing an effective reference for clinical prevention of cerebral hemorrhage transformation in patients.
