成立危重症专项护理小组对老年急性期慢性阻塞性肺疾病合并呼吸衰竭患者的临床意义

The Clinical Significance of Setting up a Special Nursing Team for Critical Care in Elderly Patients with Acute Exacerabation of Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

目的:分析危重症专项护理小组成立于老年急性期慢性阻塞性肺疾病(AECOPD)合并呼吸衰竭患者中的效果.方法:收集本院2017年12月至2020年9月收治的108例AECOPD合并呼吸衰竭患者的临床资料.根据不同护理方法分为研究组(n=56例,危重症专项护理小组干预)与对照组(n=52例,常规护理).比较两组血气指标、机械通气、ICU停留及住院时间.结果:经治疗后,两组患者其PaO_(2)、SaO_(2)水平均较治疗前升高,PaCO_(2)水平较治疗前降低,且研究组升高/降低程度均优于对照组(P<0.05).研究组机械通气、ICU停留及住院时间均低于对照组(P<0.05).结论:成立危重症专项护理小组予以特殊护理干预,对于老年AECOPD合并呼吸衰竭患者,可有效改善患者血气指标,缩短机械通气、ICU停留及住院时间.

Objective:To analyze the effect of establishing a special nursing team for critical care in elderly patients with acute exacerabation of chronic obstructive pulmonary disease(AECOPD)complicated with respiratory failure.Methods:The clinical data of 108 patients with AECOPD complicated with respiratory failure admitted to our hospital from December 2017 to September 2020 were collected.According to different nursing methods,they were divided into a study group(n=56 cases,critical care group intervention)and a control group(n=52 cases,routine care).The blood gas indexes,the time of mechanical ventilation,ICU stay and hospital stay were compared between the two groups.Results:After treatment,the levels of PaO_(2)and SaO_(2)in both two groups were higher than those before treatment,and the levels of PaCO_(2)in the two groups were lower than those before treatment.The increase or decrease level of the study group was greater than that of the control group(P<0.05).The mechanical ventilation,ICU stay and hospitalization time in the study group were lower than those in the control group(P<0.05).Conclusion:The establishment of a special nursing group for critically ill patients in elderly patients with AECOPD and respiratory failure can effectively improve the blood gas indicators of the patients,and shorten the time of mechanical ventilation,ICU stay and hospitalization.
