瘢痕子宫再次妊娠孕妇发生前置胎盘的影响因素

Influencing Factors of Occurrence of Placenta Previa in Re-Pregnant Women with Scar Uterus

目的:探究瘢痕子宫再次妊娠孕妇发生前置胎盘的影响因素.方法:采集2019年5月至2021年4月鄢陵县劳动医院收治的43例瘢痕子宫再次妊娠发生前置胎盘的孕妇资料,纳入发生组;另收集同期43例瘢痕子宫再次妊娠未发生前置胎盘的孕妇资料,纳入未发生组.查阅孕妇临床资料,将可能的影响因素纳入,分析导致瘢痕子宫再次妊娠孕妇发生前置胎盘的相关因素.结果:两组孕妇剖宫产次数、再次妊娠间隔时间、产褥感染史比例比较,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,剖宫产次数≥2次、再次妊娠间隔时间短、有产褥感染史是瘢痕子宫再次妊娠孕妇发生前置胎盘的独立危险因素(OR=3.363、20.251、6.212,P=0.019、0.002、0.024).结论:瘢痕子宫再次妊娠孕妇发生前置胎盘可能与剖宫产次数≥2次、再次妊娠间隔时间短、有产褥感染史等因素有关,临床可据此提出针对性干预方案以减少前置胎盘的发生.

Objective:To explore the influencing factors of occurrence of placenta previa in re-pregnant women with scar uterus.Methods:The data of 43 re-pregnant women with scar uterus who had placenta previa were collected from May 2019 to April 2021 in the Yanling County Labor Hospital,as the occurrence group;in addition,the data of 43 re-pregnant women with scar uterus who did not develop placenta previa during the same period were collected as the non-occurrence group.Checked the clinical data of pregnant women,and included possible influencing factors,then analyzed and found out the related factors leading to occurrence of placenta previa in re-pregnant women with scar uterus.Results:Patients in the occurrence group had statistically significant differences(P<0.05)in the times of cesarean section,interval of re-pregnancy and history of puerperal infection compared with the non-occurrence group;through Logistic regression analysis,the results showed that,times of cesarean sections≥2,the short interval of re-pregnancy and the history of puerperal infection were the independent risk factors of occurrence of placenta previa in re-pregnant women with scar uterus(OR=3.363,20.251,6.212;P=0.019,0.002,0.024).Conclusions:Occurrence of placenta previa in re-pregnant women with scar uterus may be related to the times of cesarean sections≥2,the short interval of re-pregnancy,and a history of puerperal infection,the clinical intervention program can be proposed put forward in order to reduce the incidence of placenta previa.
