人工晶体Ⅰ期与Ⅱ期植入术对外伤性白内障的临床观察及术后泪膜稳定性对比研究

摘要 目的:探究人工晶体(IOL)Ⅰ期与Ⅱ期植入术在改善外伤性白内障的临床表现及在术后泪膜稳定性方面有无差别.方法:选择本院在2017年1月至2020年12月收治的80例(80只眼睛)外伤性白内障患者,采用随机数字表法将其分为两组,各40例.研究组患者行IOLⅡ期植入术治疗,对照组患者行IOLⅠ期植入术治疗.观察对比两组患者术中出血量、手术时间、住院时间和术后视力情况,同时对两组患者术后并发症和泪膜稳定性进行对比分析.结果:对照组的出血量更多、手术时间与泪腺分泌试验时间更长,但两组患者术中住院时间、并发症发生率、术后视力、泪膜破裂时间及角膜荧光素染色评分比较,差异均无统计学意义(P>0.05).结论:IOLⅡ期植入术能够显著缩短外伤性白内障患者的手术时间,减少出血量,但对治疗效果无影响,临床上可以考虑直接进行IOLⅠ期手术. Objective:To compare the tear film stability of traumatic cataract patients after intraocular lens(IOL)implantation in stage I and stage Ⅱ.Methods:Eighty traumatic cataract patients(80 eyes)were selected from September 2017 to April 2020 in our hospital.All patients were equally divided into two groups by random number table method.The study group was treated with IOL implantation in stage Ⅱ,and the control group was treated with IOL implantation in stage I The amount of bleeding,operation time,hospitalization time,visual acuity,complications and tear film stability were compared between the two groups.Results:The control group had more bleeding,longer operation time and longer Schirmer test.There were no differences in hospital stay,complication rate,visual acuity,tear film rupture time and corneal fluorescein staining score between the two groups(P<0.05).Conclusion:IOL implantation in stage Ⅱcan significantly shorten the operation time and reduce bleeding in traumatic cataract patients,but it has no effect on the treatment effect.In clinic practice,stage I surgery can be considered directly.
