TVS、TAS在早期异位妊娠中诊断价值的对比研究

Comparative Study on the Diagnostic Value of TVS and TAS in Early Ectopic Pregnancy

目的:探讨和比较经阴道超声检查(TVS)和经腹部超声检查(TAS)在早期异位妊娠中的诊断价值.方法:选取2020年3月至2021年3月天津市黄河医院与新疆军区总医院妇科收治的91例异位妊娠患者,患者均进行TVS和TAS两种检查,对比TVS和TAS两组检查结果.结果:TVS检查异位妊娠检出率(98.90%)高于TAS(92.31%)(P<0.05);比较TVS和TAS的检查早期异位妊娠的结果,TVS检出附件存在包块、胎心、胎芽、假孕囊和卵黄囊的检出率显著高于TAS,差异具有统计学意义(P<0.05);盆腔积液检出率相当,差异无统计学意义(P>0.05).TVS对输卵管妊娠检出率高于TAS检查(P<0.05),而两种方法对卵巢妊娠、子宫角妊娠、残角子宫妊娠的检出率比较,差异无统计学意义(P>0.05).结论:TVS和TAS均能对异位妊娠进行有效的诊断,临床中应根据患者自身情况合理选择检测方式,以达到最佳诊断效果.

Objective:To study transvaginal ultrasonography(TVS)and transabdominal sonography(TAS)diagnostic value in the early ectopic pregnancy.Methods:91 patients with ectopic pregnancy admitted to Tianjin Huanghe Hospital and Xinjiang Military Region General Hospital from March 2020 to March 2021 were selected.All patients underwent TVS and TAS tests,and the results of TVS and TAS tests were compared.Results:The detection rate of TVS(98.90%)was higher than TAS(92.31%)(P<0.05).The results of early ectopic pregnancy were compared between TVS and TAS.The detection rate of accessory mass,fetal heart,fetal bud,pseudopregnancy sac and yolk sac detected by TVS was significantly higher than TAS,and the difference was statistically significant(P<0.05),while the detection rate of pelvic effusion was similar,and the difference was not statistically significant(P>0.05).The detection rate of tubal pregnaney by TVS was higher than that by TAS(P<0.05),and there was no significant difference between the two methods in the detection rate of ovarian pregnancy,uterine horn pregnancy and residual horn pregnancy(P>0.05).Conclusion:TVS and TAS can effectively diagnose ectopic pregnancy.In clinical practice,reasonable selection of detection methods should be made according to the patient's own situation to achieve the best diagnostic effect.
