健侧卧奔跑位智能控压输尿管镜吸引取石术治疗输尿管上段及肾盂结石的临床疗效

Clinical Efficacy of Intelligent Pressure-Controlled Ureteroscopic Lithotomy in the Treatment of Upper Ureteral and Renal Pelvic Calculi

目的探讨健侧卧奔跑位智能控压输尿管镜吸引取石术治疗输尿管上段及肾盂结石的临床效果.方法回顾性收集2020年1月至2021年2月在赣州市人民医院接受健侧卧奔跑位下智能控压输尿管镜吸引取石手术的212例输尿管上段及肾盂结石患者的临床资料.统计分析212例手术患者临床数据,包括手术成功率,成功施行手术患者的一期清石率、平均手术时间、术后住院时间及并发症发生情况.结果212例患者中,成功施行健侧卧奔跑位智能控压输尿管镜吸引取石术207例,手术成功率为97.64%(207/212).207例患者一期清石率97.58%(202/207),平均手术时间为(23.43±12.61)min.术后平均住院天数为(1.61±0.74)d.术后出现较明显的肉眼血尿12例;术后出现发热3例,经加强抗感染治疗后恢复良好;术后腰疼明显5例,考虑支架管不耐受导致,予以解痉止痛等对症治疗后症状缓解.结论健侧卧奔跑位智能控压输尿管镜吸引取石术治疗输尿管上段及肾盂结石具有手术成功率高,清石效果好,并发症发生率低等优势.

Objective To explore the clinical effect of intelligent pressure-controlled ureteroscope aspiration and removal of stones in the contralateral lying running position in the treatment of upper ureteral and renal pelvic calculi.Methods The clinical data of 212 patients with upper ureteral and renal pelvic stones were retrospectively collected in Ganzhou People's Hospital from January 2020 to February 2021.The clinical data of 212 surgical patients were statistically analyzed,including the success rate of the operation,the rate of one-stage stone removal of the successfully performed operation,the average operation time,the postoperative hospital stay and the occurrence of complications.Results Among the 212 patients,207 cases were successfully performed the intelligent pressure-controlled ureteroscopy on the contralateral side and running position,and the operation success rate was 97.64%(207/212).The first-stage stone removal rate of 207 patients was 97.58%(202/207),and the average operation time was(23.43±12.61)min.The average postoperative hospital stay was(1.61±0.74)days.12 cases had obvious gross hematuria after operation;3 cases had fever after operation,and recovered well after intensive anti-inflammatory treatment;5 cases had obvious back pain after operation,considering the stent intolerance,after symptomatic treatment such as antispasmodic analgesia the symptoms are relieved.Conclusion Intelligent pressure-controlled ureteroscopy for the treatment of upper ureter and renal pelvis stones with the contralateral lying running position has the advantages of high surgical success rate,good stone removal effect,and low complication rate.
