重症患者的营养评估引导下个性化营养支持护理研究

Study on individualized nutritional support nursing guided by nutritional assessment for patients with severe diseases

目的探讨营养评估个性化营养支持护理对重症患者的干预效果.方法选取116例ICU重症患者作为研究对象,采用随机数表法将其分成对照组与观察组,各58例.对照组采用ICU重症常规护理,观察组在常规护理基础上采用营养评估引导的个性化营养支持护理.观察并比较2组患者的上臂肌围(MAMC)、三头肌皮褶厚度(TSF)、体质量指数(BMI)等体格指标和淋巴细胞计数(LY)、血红蛋白(Hb)、超敏C反应蛋白(hs-CRP)、前白蛋白(PALB)、总蛋白(TP)、血浆白蛋白(ALB)等血生化与血常规指标变化情况.结果护理后,观察组的TSF、BMI、hs-CRP、PALB、TP、ALB、MAMC、LY、Hb指标水平均优于对照组,差异有统计学意义(P<0.05);观察组的并发症发生率低于对照组,差异有统计学意义(P<0.05).结论采用营养评估引导的个性化营养支持护理对重症患者进行护理,能明显改善患者营养状况,降低并发症发生率,促进疾病康复.

Objective To explore the intervention effect of individualized nutritional support nursing guided by nutritional assessment for patients with severe diseases.Methods A total of 116 patients in ICU were selected and randomly divided into control group(n=58)and observation group(n=58).The control group was conducted with intensive routine nursing of ICU,while the observation group was treated with individualized nutritional support nursing guided by nutrition assessment on the basis of the routine nursing.The changes of physical indexes such as upper arm muscle circumference(MAMC),triceps skinfold thickness(TSF)and body mass index(BMI)as well as blood biochemical and blood routine indexes such as lymphocyte count(LY),hemoglobin(Hb),high-sensitivity C reactive protein(hs-CRP),pre-albumin(PALB),total protein(TP)and plasma albumin(ALB)were observed and compared between two groups.Results After nursing,the levels of TSF,BMI,hs-CRP,PALB,TP,ALB,MAMC,LY and Hb in the observation group were significantly better than those in the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion For patients with severe diseases,individualized nutritional support nursing guided by nutritional assessment can significantly improve the nutritional status,reduce the incidence of complications and promote recovery of diseases.
