摘要
目的:研究胃癌高发区山西省幽门螺旋杆菌(Hp)感染与血清胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、PGⅠ/PGⅡ(PGR)以及胃泌素-17(G-17)浓度的相关性.方法:选取2019年7月1日至12月31日山西医科大学第一医院健康管理中心参与胃癌筛查项目的体检人群为研究对象,剔除无14C呼气试验493例,血清胃功能检测不全1例,最终纳入2547例,检测G-17、PGⅠ、PGⅡ、PGR,并行14C呼气试验检测,比较分析G-17、PGⅠ、PGⅡ、PGR及Hp在不同性别及年龄段的差异及变化.结果:Hp感染率是34.93%,其中男性感染率是63.92%,女性感染率是33.06%;Hp感染率是34.93%,其中男性感染率是63.92%,女性感染率是33.06%,不同性别Hp感染率比较,差异无统计学意义(χ2=2.581,P>0.05);男性PGⅠ、PGⅡ、PGR浓度水平较女性均高,男性G-17浓度水平较女性低,不同性别G-17、PGⅠ、PGⅡ及PGR比较,差异有统计学意义(P<0.05);HP阳性组的血清PGⅠ、PGⅡ及G17浓度水平均高于阴性组,PGR低于阴性组,差异有统计学意义(P<0.05);PGⅠ、PGⅡ及G17浓度水平随着年龄段的增长而升高,5组间差异有统计学意义(P<0.05),PGR浓度水平40~49岁组较小于40岁组有微小的升高,但总体水平呈随年龄段的增长而降低的趋势,5组间差异有统计学意义(P<0.05).结论:Hp阳性者中血清G-17、PGⅠ及PGⅡ浓度较阴性者高,血清G-17、PGⅠ及PGⅡ浓度随着年龄的增长而增高,应密切关注Hp阳性的中老年者血清G-17、PGⅠ、PGⅡ浓度及PGR的变化.
Objective:Study of gastric cancer high incidence of Shanxi Province helicobacter pylori(Hp)infection and serum pepsinogenⅠ(PGⅠ),pepsinogenⅡ(PGⅡ),PGⅠ/PGⅡ(PGR)and gastric secrete element-17(G-17)concentration of relevance.Methods:Select 1 July 2019 to December 31,the first hospital of shanxi medical university health management center to participate in gastric cancer screening project check-up crowd as the research object,and 493 cases without 14C breath test,serum gastric function testing is not complete in 1 case,finally in 2547 cases,detection of G-17,PGⅠ,PGⅡ,PGR,exhale parallel 14C test,comparative analysis of G-17,PGⅠ,PGⅡ,PGR,and Hp in different gender and age differences and changes.Results:Hp infection rate was 34.93%,among which male infection rate was 63.92%and female infection rate was 33.06%.The Hp infection rate was 34.93%,among which the male infection rate was 63.92%and the female infection rate was 33.06%.The difference of Hp infection rate between different genders was not statistically significant(χ2=2.581,P>0.05).Male PGⅠ,PGⅡ,high concentration of PGR levels are more than female,male G17 concentration level was lower than those of women,different gender G-17,PGⅠ,PGⅡand PGR comparison,the difference was statistically significant(P<0.05);Serum PGⅠ,PGⅡand G17 concentrations in the HP positive group were higher than those in the negative group,and PGR was lower than that in the negative group,with statistically significant differences(P<0.05).The concentration levels of PGⅠ,PGⅡand G17 increased with the increase of age groups,and the difference between the five groups was statistically significant(P<0.05).The PGR concentration level of 40-49 years old group was slightly higher than that of the group less than 40 years old group,but the overall level showed a trend of decrease with the increase of age groups,and the difference between the five groups was statistically significant.Conclusion:In the Hp positive serum G-17,PGⅠand PGⅡconcentration more negatie,high serum G-17,PGⅠand PGⅡconcentration increased with age,should pay close attention to the Hp positive middle-aged and old G-17,serum PGⅠ,PGⅡconcentration and the change of PGR.
