基于发展性照护的多维护理对肠造口新生儿疼痛效果的研究

Effect of multi-dimensional nursing based on developmental care on pain in neonates with enterostomy

目的探讨基于发展性照护的多维护理对肠造口新生儿疼痛的应用效果。方法选取2021年1月至2022年12月99例肠造口新生儿作为研究对象,按照入院时间分为观察组(n=50)和对照组(n=49)。对照组按照常规造口护理,观察组在对照组的基础上实施基于发展性照护的多维护理,包括鸟巢护理、白噪声、非营养性吸吮、甜味剂、体位及温度管理。比较两组患儿操作前、操作中、操作后的心率、血氧饱和度、疼痛评分及恢复基础心率时间。结果对照组和观察组操作前心率、血氧饱和度、疼痛评分差异无统计学意义(P>0.05);观察组操作中心率、血氧饱和度的波动幅度小于对照组,疼痛评分及恢复基础心率的时间均小于对照组,差异有统计学意义(P<0.05)。结论基于发展性照护的多维护理能够有效减少肠造口护理过程中的不良刺激,降低疼痛评分。

Objective To explore the application effect of multi-dimensional nursing based on developmental care on pain in neonates with enterostomy.Methods Ninety-nine neonates with enterostomy from January 2021 to December 2022 were selected as subjects,and divided into the observation group(n=50)and the control group(n=49)according to the time of admission.The control group was treated with conventional ostomy care,while the observation group was treated with extra mult-idimensional nursing based on developmental care,including bird's nest care,white noise,non-nutritional sucking,sweetener,body position and temperature management.Heart rate,oxygen saturation,pain score and recovery time of basal heart rate were compared between the two groups before,during and after operation.Results There were no significant differences in heart rate,oxygen saturation and pain score between the control group and the observation group before operation(P>0.05).The fluctuation range of heart rate and oxygen saturation in the observation group during the operation were less than those in the control group.And the pain score and recovery time of basic heart rate were also less than those in the control group.The differences were statistically significant(P<0.05).Conclusion Multi-dimensional nursing based on developmental care can effectively reduce adverse irritation and pain score during enterostomy nursing.
