项针联合康复训练对脑卒中后吞咽障碍患者吞咽功能的疗效观察

Effect of Neck Acupuncture Combined with Rehabilitation Training on Swallowing Function in Patients with Dysphagia after Stroke

目的探讨项针联合康复训练对脑卒中后吞咽障碍患者吞咽功能的疗效.方法选取我院2020年6月—2023年6月收治的176例脑卒中后吞咽障碍患者为研究对象,按照随机数字表法将其分为对照组与观察组,各88例.对照组采用康复训练干预,观察组在对照组基础上联合项针干预.比较两组患者的吞咽功能、生活质量及咽侧壁运动参数.结果干预前,两组功能性经口摄食量表(FOIS)、吞咽特异性生活质量量表(SWAL-QQL)、标准吞咽功能评估量表(SSA)评分及咽侧壁运动参数对比,组间差异无统计学意义(P>0.05);干预后,观察组FOIS、SWAL-QQL评分均高于对照组,咽侧壁运动幅度大于对照组,运动速度快于对照组,SSA评分低于对照组,运动时间短于对照组,组间差异有统计学意义(P<0.05).结论与单独使用康复训练相比,脑卒中后吞咽障碍患者加用项针干预能够提升整体效果,改善患者吞咽功能,增强咽侧壁运动能力,提高患者生活质量,值得推广应用.

Objective To investigate the effect of neck acupuncture combined with rehabilitation training on swallowing function in patients with dysphagia after stroke.Methods 176 patients with dysphagia after stroke admitted to our hospital from June 2020 to June 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 88 cases in each group.The control group received rehabilitation training intervention,and the observation group was combined with neck acupuncture intervention on the basis of the control group.Swallowing function,quality of life and movement parameters of pharyngeal lateral wall were compared between the two groups.Results Before intervention,the Functional Oral Intake Scale(FOIS),Swallowing Specific Quality of Life Scale(SWAL-QQL),Standardized Swallowing Assessment(SSA)scores,and pharyngeal lateral wall movement parameters,and there were no statistical significance between the two groups(P>0.05);after the intervention,the FOIS and SWAL-QQL scores in the observation group were higher than those in the control group,the amplitude of lateral pharyngeal wall movement was greater than that in the control group,the movement speed was faster than that in the control group,the SSA score was lower than that in the control group,and the exercise time was shorter than that in the control group,with statistical significance(P<0.05).Conclusion Compared with rehabilitation training alone,neck acupuncture intervention in patients with swallowing disorder after stroke can improve the overall effect,improve the swallowing function of patients,enhance the movement ability of pharyngeal side wall,and improve the quality of life of patients,which is worthy of popularization and application.
