马来酸麦角新碱联合缩宫素治疗子宫收缩乏力性产后出血患者的临床效果观察

Clinical Observation of Ergometrine Maleate Combined with Oxytocin in the Treatment of Postpartum Hemorrhage Caused by Uterine Atony Postpartum Hemorrhage

目的探究马来酸麦角新碱联合缩宫素治疗在子宫收缩乏力性产后出血患者中的应用价值.方法选取我院于2021年5月—2023年8月收治的94例子宫收缩乏力性产后出血患者为研究对象,使用随机数字表法将其分为对照组与观察组,每组47例.对照组使用缩宫素治疗,观察组在使用缩宫素治疗的同时增加马来酸麦角新碱进行治疗.比较两组止血效果、出血情况、宫缩次数、宫底高度、凝血功能指标及不良反应发生情况.结果观察组止血总有效率比对照组高,产后各时间段总出血量均比对照组少,用药30 min、1 h后的宫缩次数均较对照组多,宫底高度均较对照组低,组间差异有统计学意义(P<0.05);治疗后,观察组活化部分凝血活酶时间、凝血酶原时间及凝血酶时间均比对照组短,组间差异有统计学意义(P<0.05);且两组不良反应总发生率比较,差异无统计学意义(P>0.05).结论马来酸麦角新碱、缩宫素联合治疗子宫收缩乏力性产后出血患者,可有效提高止血效果,增加宫缩次数,降低宫底高度,改善凝血功能,且安全性高.

Objective To explore the application value of ergometrine maleate combined with oxytocin in the treatment of postpartum hemorrhage with uterine atony postartum hemorrhage.Methods A total of 94 patients with uterine atony postartum hemorrhage admitted to our hospital from May 2021 to August 2023 were selected as the research objects.They were divided into a control group and an observation group by the random number table method,with 47 cases in each group.The control group was treated with oxytocin,and the observation group was treated with ergometrine maleate in addition to oxytocin.The hemostatic effect,bleeding,uterine contraction times,height of fundus,coagulation function indexes and adverse reactions were compared between the two groups.Results The total effective rate of hemostasis in the observation group was higher than that in the control group,the total amount of postpartum bleeding in each time period was less than that in the control group,the number of uterine contractions after 30 min and 1 h of medication was more than that in the control group,and the height of uterine fundus was lower than that in the control group,the differences between the two groups were statistically significant(P<0.05).After treatment,the activated partial thromboplastin time,prothrombin time and thrombin time in the observation group were shorter than those in the control group,and the differences between the two groups were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Ergometrine maleate combined with oxytocin in the treatment of postpartum hemorrhage patients with uterine atony postartum hemorrhage can effectively improve the hemostatic effect,increase the number of contractions,reduce the height of fundus,improve coagulation function,and has high safety.
