发展性照顾护理结合袋鼠式护理对早产儿体格发育指标、神经行为发育的影响

The Impact of Developmental Care Combined with Kangaroo Style Care on Physical Development Indicators and Neurobehavioral Development of Premature Infants

目的探究发展性照顾护理结合袋鼠式护理对早产儿体格发育指标、神经行为发育的影响。方法选择2021年2月—2023年6月我院收治的78例早产儿为研究对象,按随机数字表法将其分为对照组与观察组,各39例。两组早产儿均采用常规护理,对照组在常规护理基础上采用早产儿袋鼠式护理,观察组在对照组基础上采用发展性照顾护理。对比两组早产儿体格发育指标、神经行为发育和免疫功能。结果出生14 d,观察组体质量增长重于对照组,身高增长高于对照组,头围增长大于对照组,组间差异有统计学意义(P<0.05);出生30 d,观察组体质量增长重于对照组,身高增长高于对照组,头围增长大于对照组,组间差异有统计学意义(P<0.05);干预后,观察组新生儿神经行为测定量表评分高于对照组,差异有统计学意义(P<0.05);干预后,观察组成熟的T淋巴细胞亚群CD3^(+)、CD4^(+)水平均高于对照组,CD8^(+)水平低于对照组,组间差异有统计学意义(P<0.05)。结论发展性照顾护理结合袋鼠式护理有助于早产儿的体格发育,并促进早产儿神经行为发育,提高其免疫能力,值得推广。

Objective To explore the effects of developmental nursing combined with kangaroo nursing on physical development indexes and neurobehavioral development of premature infants.Methods 78 premature infants admitted to our hospital from February 2021 to June 2023 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 39 cases in each group.Both groups were given routine nursing care,the control group was given kangaroo nursing care based on routine nursing,and the observation group was given developmental nursing care based on the control group.Physical development indexes,neurobehavioral development and immune function were compared between the two groups.Results 14 days after birth,the body mass,height and head circumference of observation group increased more than control group,and the differences between groups were statistically significant(P<0.05).At 30 days after birth,body mass,height and head circumference of observation group increased more than control group,and the differences between groups were statistically significant(P<0.05);after intervention,the score of neonatal neurobehavioral scale in observation group was higher than that in control group,and the difference was statistically significant(P<0.05);after intervention,the levels of mature T lymphocyte subsets CD3^(+)and CD4^(+)in the observation group were higher than those in the control group,and the levels of CD8^(+)were lower than those in the control group,with statistical significance(P<0.05).Conclusion Developmental nursing combined with kangaroo nursing can contribute to the physical development of preterm infants,promote the neurobehavioral development of preterm infants,and improve their immune capacity,which is worth popularizing.
