徒手淋巴引流术联合多层弹力绷带加压包扎应用于创伤后继发性淋巴水肿患者中的效果

Effect of Manual Lymphatic Drainage Combined with Multi-Layer Elastic Bandage Compression Bandaging in Patients with Secondary Lymphedema after Trauma

目的探究徒手淋巴引流术联合多层弹力绷带加压包扎应用于创伤后继发性淋巴水肿患者中的效果。方法选择2021年7月—2023年7月铜仁市人民医院收治的86例创伤后下肢肿胀患者为研究对象,按照随机数字表法将其分为对照组和观察组,每组43例。对照组仅行徒手淋巴引流术,观察组在此基础上采用多层弹力绷带加压包扎,均治疗2周。比较两组患者的临床疗效、患肢周径及疼痛程度。结果观察组的治疗总有效率为95.35%,高于对照组的81.40%,差异有统计学意义(P<0.05)。治疗后,观察组的髌骨上缘上20 cm、髌骨上缘上10 cm、髌骨下缘下10 cm、髌骨下缘下20 cm、足中周径均短于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的数字疼痛强度量表评分低于对照组,差异有统计学意义(P<0.05)。结论徒手淋巴引流术联合多层弹力绷带加压包扎应用于创伤后下肢肿胀患者中的效果显著,可减轻患肢肿胀、疼痛,加快恢复进程。

Objective To investigate the effect of manual lymphatic drainage combined with multi-layer elastic bandage compression bandaging in the treatment of secondary lymphedema after trauma.Methods 86 patients with post-traumatic lower limb swelling admitted to Tongren People's Hospital from July 2021 to July 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 43 cases in each group.The control group received only manual lymphatic drainage,and the observation group was treated with multi-layer elastic bandage compression bandaging on this basis.Both groups were treated for 2 weeks.The clinical efficacy,circumference of the affected limb and pain degree of the two groups were compared.Results The total effective rate of the observation group was 95.35%,higher than 81.40%of the control group,the difference was statistically significant(P<0.05).After treatment,the diameters at 20 cm above the upper margin of the patella,10 cm above the upper margin of the patella,10 cm below the lower margin of the patella,20 cm below the lower margin of the patella,and the middle circumference of the foot of the observation group were shorter than those of the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the pain numerical rating scale score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of manual lymphatic drainage combined with multi-layer elastic bandage compression bandaging in the treatment of patients with lower limb swelling after trauma is effective,which can reduce the swelling and pain of the affected limb and accelerate the recovery process.
