地屈孕酮联合黄体酮对黄体功能不全先兆流产患者症状缓解时间及保胎结局的影响

Effect of Dydrogesterone Combined with Progesterone on Symptom Remission Time and Fetal Saving Outcome in Patients with Threatened Abortion with Luteal Insufficiency

目的探究地屈孕酮联合黄体酮对黄体功能不全先兆流产患者症状缓解时间及保胎结局的影响。方法选取2020年10月—2022年8月日照市东港区妇幼保健院收治的84例黄体功能不全先兆流产患者为研究对象,按随机数字表法将其分为对照组及观察组,每组42例。对照组采用黄体酮胶丸治疗,观察组在此基础上采用地屈孕酮片治疗,均持续治疗2周。比较两组患者的孕激素水平、症状缓解时间及保胎结局。结果治疗后,观察组的雌二醇、孕酮、人绒膜促性腺激素、孕激素诱导阻断因子水平均高于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的止血时间、腰部酸痛改善时间、腹部疼痛改善时间均短于对照组,组间差异有统计学意义(P<0.05)。观察组的流产率为9.52%,低于对照组的28.57%,差异有统计学意义(P<0.05)。结论黄体功能不全先兆流产患者采用地屈孕酮联合黄体酮治疗的效果显著,能缩短症状缓解时间,降低流产率,并有效调控孕激素水平,值得临床推广使用。

Objective To explore the effects of dydrogesterone combined with progesterone on symptom remission time and fetal saving outcome in patients with threatened abortion with luteal insufficiency.Methods 84 patients with threatened abortion with luteal insufficiency admitted to Donggang District Maternal and Child Health Care Hospital of Rizhao City from October 2020 to August 2022 were selected as the study objects,and were divided into a control group and an observation group according to random number table method,with 42 cases in each group.The control group was treated with progesterone capsules,and the observation group was treated with desdrogesterone tablets on this basis.Both groups were treated for 2 weeks.The levels of progesterone,the duration of symptom remission and the outcome of fetal preservation were compared between the two groups.Results After treatment,the levels of estradiol,progesterone,human chorionic gonadotropin and progesterone induced blocking factor in the observation group were higher than those in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the hemostatic time,lumbar soreness improvement time and abdominal pain improvement time in the observation group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).The abortion rate in the observation group was 9.52%,which was lower than 28.57%in the control group,and the difference was statistically significant(P<0.05).Conclusion Dedrogesterone combined with progesterone can shorten the time of symptom relief,reduce the rate of abortion,and effectively regulate the level of progesterone in patients with threatened abortion with luteal insufficiency,which is worthy of clinical promotion.
