复合保温措施在腹腔镜胆囊切除术患者中的应用效果观察

Obervation of the Effects of Composite Insulation Measures in Patients Undergoing Laparoscopic Cholecystectomy

目的分析腹腔镜胆囊切除术(LC)开展过程中复合保温措施的应用效果。方法选取2021年1月—2022年12月于盐城市亭湖区人民医院行LC手术治疗的100例患者为研究对象,按随机数字表法将其分为对照组及研究组,每组50例。对照组采用常规保温处理,研究组采用复合保温干预。对比两组患者的体温、生理指标及凝血功能。结果气腹建立后10 min、手术结束放气后5 min、手术结束3 h,研究组的鼻咽温度均高于对照组,组间差异有统计学意义(P<0.05)。术后,研究组的心率、舒张压、收缩压、去甲肾上腺素水平、多巴胺水平均低于对照组,组间差异有统计学意义(P<0.05)。术后,研究组的纤维蛋白原水平为(2.31±0.32)g/L,低于对照组,凝血酶时间、凝血酶原时间分别为(12.13±1.31)s、(14.36±1.77)s,均短于对照组,组间差异有统计学意义(P<0.05)。结论LC手术过程中采用复合保温干预的效果理想,有助于维持患者术中生命体征稳定,保障手术安全,值得临床推广使用。

Objective To analyze the effects of composite insulation during laparoscopic cholecystectomy(LC).Methods 100 patients who underwent LC surgery in Tinghu People's Hospital of Yancheng City from January 2021 to December 2022 were selected as the study objects,and were divided into a control group and a study group according to random number table method,with 50 cases in each group.The control group was treated with conventional thermal insulation,and the study group was treated with composite thermal insulation.The body temperature,physiological indexes and coagulation function of the two groups were compared.Results The nasopharyngeal temperatures of the study group were higher than those of the control group at 10 min after the establishment of pneumoperitoneum,5 min after the end of operation and 3 h after the end of operation,and the differences between the groups were statistically significant(P<0.05).After operation,the heart rate,diastolic blood pressure,systolic blood pressure,norepinephrine level and dopamine level of the study group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).After surgery,the fibrinogen level of the study group was(2.31±0.32)g/L,lower than that of the control group,and the thrombin time and prothrombin time were(12.13±1.31)s and(14.36±1.77)s,respectively,shorter than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The effect of composite insulation intervention during LC surgery is ideal,which is helpful to maintain the stability of patients'vital signs and ensure the safety of surgery,and is worthy of clinical promotion.
