基于心肺运动试验导向的心脏康复疗法结合抗阻训练治疗慢性心力衰竭对患者心功能的影响

The Impact of Cardiac Rehabilitation Therapy Combined with Resistance Training Guided by Cardiopulmonary Exercise Test on the Cardiac Function of Patients with Chronic Heart Failure

目的探讨基于心肺运动试验导向的心脏康复疗法结合抗阻训练在慢性心力衰竭(CHF)患者中的应用效果。方法选取2021年5月—2023年5月我院收治的70例CHF患者为研究对象,按随机数字表法将其分为对照组与观察组,各35例。对照组采用抗阻训练,观察组在对照组治疗基础上加用基于心肺运动试验导向的心脏康复疗法。对比两组心功能指标、运动耐力、心脏不良事件发生情况及生活质量。结果治疗后,观察组左室射血分数高于对照组,左心室舒张末期内径、左心室收缩末期内径均短于对照组,明尼苏达心力衰竭生命质量量表中的身体活动、情感状况、症状评分均低于对照组,6 min步行距离长于对照组,组间差异有统计学意义(P<0.05)。心脏不良事件发生率为5.71%,低于对照组的22.85%,差异有统计学意义(P<0.05)。结论基于心肺运动试验导向的心脏康复疗法结合抗阻训练能够改善CHF患者心功能与运动耐力,减少心脏不良事件,提高生活质量。

Objective To explore the effect of cardiopulmonary exercise test-oriented cardiac rehabilitation combined with resistance training in patients with chronic heart failure(CHF).Methods 70 CHF patients admitted to our hospital from May 2021 to May 2023 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 35 cases in each group.Patients in the control group were treated with resistance training,and patients in the observation group were treated with cardiopulmonary exercise test-oriented cardiac rehabilitation therapy on the basis of the treatment in the control group.The cardiac function index,exercise endurance,adverse cardiac events and quality of life were compared between the two groups.Results After treatment,the left ventricular ejection fraction of the observation group was higher than that of the control group,the left ventricular end-diastolic diameter and left ventricular end-systolic diameter were shorter than those of the control group,the physical activity,emotional status and symptom scores of the Minnesota Quality of Life with Heart Failure scale were lower than those of the control group,and the 6-minute walk distance was longer than that of the control group,the differences between the two groups were statistically significant(P<0.05).The incidence of cardiac adverse events in the observation group was 5.71%,which was lower than 22.85%in the control group,and the difference was statistically significant(P<0.05).Conclusion Cardiac rehabilitation therapy based on cardiopulmonary exercise testing combined with resistance training can improve cardiac function and exercise endurance of CHF patients,reduce cardiac adverse events,and improve the quality of life.
