补气活血汤配合火龙罐治疗寒湿痹阻型老年腰椎间盘突出症的效果

Effect of Buqi Huoxue Decoction Combined with Huolong Pot in the Treatment of Elderly Lumbar Disc Herniation with Cold Dampness Obstruction Syndrome

目的探讨补气活血汤配合火龙罐治疗寒湿痹阻型老年腰椎间盘突出症(LDH)的效果。方法选取2022年5月—2023年5月淄博市张店区中医院收治的82例寒湿痹阻型老年LDH患者为研究对象,按随机数字表法将其分为对照组及观察组,每组41例。对照组采用塞来昔布治疗,观察组在此基础上采用补气活血汤配合火龙罐治疗,均持续治疗4周。比较两组患者的疼痛评分、Oswestry功能障碍指数(ODI)评分、腰椎功能、血清学指标及不良反应发生情况。结果治疗后,观察组的疼痛视觉模拟评分、ODI评分分别为(1.54±0.23)分、(12.31±1.24)分,均低于对照组的(1.89±0.26)分、(16.84±1.41)分,组间差异有统计学意义(P<0.05)。治疗后,观察组的日本骨科协会评估治疗分数中主观症状、临床体征、日常活动受限评分及总分分别为(7.45±1.13)分、(4.94±0.52)分、(12.13±1.12)分、(24.52±2.14)分,均高于对照组的(6.02±1.04)分、(3.87±0.43)分、(10.41±1.05)分、(20.30±2.11)分,组间差异有统计学意义(P<0.05)。治疗后,观察组的前列腺素E2、C反应蛋白水平分别为(105.63±10.12)pg/mL、(5.14±1.02)mg/L,均低于对照组的(117.65±11.43)pg/mL、(6.89±1.14)mg/L,β-内啡肽水平为(186.95±15.78)pg/mL,高于对照组的(165.74±14.39)pg/mL,组间差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论补气活血汤配合火龙罐治疗寒湿痹阻型老年LDH的效果显著,可降低血清学指标水平,减轻局部疼痛,加快腰椎功能恢复,安全可靠。

Objective To investigate the effect of Buqi Huoxue Decoction combined with Huolong Pot in the treatment of elderly lumbar disc herniation(LDH)with cold dampness obstruction syndrome.Methods 82 elderly LDH patients with cold and damp impediment syndrome admitted to Zhangdian District Hospital of Traditional Chinese Medicine of Zibo City from May 2022 to May 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 41 cases in each group.The control group was treated with celecoxib,and the observation group was treated with Buqi Huoxue Decoction combined with Huolong Pot on this basis.Both groups were treated for 4 weeks.The pain score,Oswestry Disability Index(ODI)score,lumbar function,serological indexes and incidence of adverse reactions were compared between the two groups.Results After treatment,the visual analogue scale score and ODI score of the observation group were(1.54±0.23)points and(12.31±1.24)points,respectively,which were lower than(1.89±0.26)points and(16.84±1.41)points of the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the subjective symptoms,clinical signs,daily activity limitation scores and total score of the Japanese Orthopaedic Association in the observation group were(7.45±1.13)points,(4.94±0.52)points,(12.13±1.12)points and(24.52±2.14)points,respectively,which were higher than(6.02±1.04)points,(3.87±0.43)points,(10.41±1.05)points,(20.30±2.11)points of the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of prostaglandin E2 and C-reactive protein in the observation group were(105.63±10.12)pg/mL and(5.14±1.02)mg/L,respectively,which were lower than(117.65±11.43)pg/mL and(6.89±1.14)mg/L in the control group,while the level ofβ-endorphin was(186.95±15.78)pg/mL,higher than(165.74±14.39)pg/mL in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Buqi Huoxue Decoction combined with Huolong Pot has a significant effect in the treatment of elderly LDH with cold and damp obstruction syndrome,which can reduce the level of serological indexes,relieve local pain,accelerate the recovery of lumbar function,and is safe and reliable.
