枸地氯雷他定联合鼻窦内窥镜手术对成年慢性鼻窦炎患者血清炎症指标、变态反应指标的影响

Effects of Desloratadine Citrate Combined with Endoscopic Surgery on Serum Inflammation and Allergic Response in Adult Patients with Chronic Sinusitis

目的探讨枸地氯雷他定联合鼻窦内窥镜手术对成年慢性鼻窦炎患者血清炎症指标、变态反应指标的影响。方法选择2021年5月-2023年5月邹平市人民医院收治的116例慢性鼻窦炎患者为研究对象,按随机数字表法将其分为对照组及观察组,每组58例。对照组采用鼻窦内窥镜手术治疗,观察组在此基础上采用枸地氯雷他定片治疗。对比两组患者的血清炎症指标、鼻黏液纤毛清除功能、变态反应指标及预后情况。结果治疗后,观察组的白细胞介素-6、超敏C反应蛋白、白细胞介素-8、肿瘤坏死因子-α水平均低于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的鼻黏液纤毛清除率高于对照组,鼻黏液纤毛清除速度快于对照组,糖精清除时间短于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的嗜酸粒细胞阳离子蛋白、免疫球蛋白E蛋白总量水平均低于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的鼻腔鼻窦结局测试-20评分为(15.43±0.98)分,低于对照组的(23.74±1.18)分,差异有统计学意义(P<0.05)。结论采用枸地氯雷他定联合鼻窦内窥镜手术治疗成年慢性鼻窦炎患者,可显著改善变态反应,降低炎症反应,提升其鼻黏液纤毛清除功能,提高预后,值得临床推广使用。

Objective To investigate the effects of desloratadine citrate combined with endoscopic surgery on serum inflammatory indexes and allergic response indexes in adult patients with chronic sinusitis.Methods 116 patients with chronic sinusitis admitted to Zouping People's Hospital from May 2021 to May 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 58 cases in each group.The control group received endoscopic sinus surgery,and the observation group was treated with desloratadine citrate tablets on this basis.The serum inflammatory indexes,nasal mucociliary clearance function,allergy response indexes and prognosis were compared between the two groups.Results After treatment,the levels of interleukin-6,hypersensitive C-reactive protein,interleukin-8 and tumor necrosis factor-α in the observation group were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the clearance rate of nasal mucocilia was higher than that in the control group and the clearance speed of nasal mucocilia in the observation group was faster than that in the control group,and the clearance time of saccharin was shorter than that in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the total levels of eosinophil cationic protein and total immunoglobulin E protein in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the sinus and nasal outcome test-20 score of the observation group was(15.43±0.98)points,which was lower than(23.74±1.18)points of the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of desloratadine citrate and endoscopic surgery in the treatment of adult patients with chronic sinusitis can significantly improve the allergic reaction,reduce the inflammatory response,improve the nasal mucociliary clearance function,improve the prognosis,and is worthy of clinical promotion.
