双上肢运动想象对脑卒中偏瘫患者上肢运动功能及日常生活活动能力的影响

Effect of Dual Upper Limb Motor Imagination on Upper Limb Motor Function and Activity of Daily Living in Stroke Patients with Hemiplegia

目的探讨脑卒中偏瘫患者进行双上肢运动想象训练的效果。方法选取2020年11月-2021年4月上海市第二康复医院收治的40例患者脑卒中偏瘫患者为研究对象,按随机数字表法将其分为对照组及观察组。两组均采用常规康复治疗,在此基础上,对照组(n=20)进行患侧上肢运动想象训练,观察组(n=20)进行双上肢运动想象训练,均持续4周。对比两组患者的上肢运动功能、日常生活活动能力及生活质量。结果治疗后,观察组的Fugl-Meyer肢体运动功能量表、改良Barthel指数评分分别为(47.12±5.11)分、(78.04±6.88)分,均高于对照组的(40.46±4.60)分、(71.79±6.23)分,组间差异有统计学意义(P<0.05)。治疗后,观察组的健康调查量表评分为(82.26±7.77)分,高于对照组的(75.88±6.49)分,差异有统计学意义(P<0.05)。结论双上肢运动想象训练能改善脑卒中偏瘫患者的上肢运动功能、日常生活活动能力,提升其生活质量,值得临床推广使用。

Objective To investigate the effect of dual upper limb motor imagination training on stroke patients with hemiplegia.Methods 40 patients with stroke hemiplegia admitted to Shanghai Second Rehabilitation Hospital from November 2020 to April 2021 were selected as the study objects and were divided into a control group and an observation group according to random number table method.Both groups were treated with conventional rehabilitation therapy.On this basis,the control group(n=20)underwent the affected upper limb movement imagination training,and the observation group(n=20)underwent the dual upper limb movement imagination training.Both groups were treated for 4 weeks.The upper limb motor function,activity of daily living and quality of life were compared between the two groups.Results After treatment,the scores of Fugl-Meyer assessment scale and modified Barthel index in the observation group were(47.12±5.11)points and(78.04±6.88)points,which were higher than(40.46±4.60)points and(71.79±6.23)points in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the MOS item short from health survey score of the observation group was(82.26±7.77)points,which was higher than(75.88±6.49)points of the control group,and the difference was statistically significant(P<0.05).Conclusion Dual upper limb motor imagination training can improve the upper limb motor function,daily living ability and quality of life of stroke patients with hemiplegia,which is worthy of clinical application.
