摘要
目的研究高强度术前训练对膝关节置换术后患者早期疼痛和功能恢复的影响.方法选取赤峰学院附属医院2021年1月—2023年4月间收治的76例骨关节炎(OA)膝关节置换术患者为研究对象,按照随机数字表法将其分为对照组和观察组,各38例.所有患者均接受膝关节置换术治疗,其中对照组患者术前接受常规管理,观察组患者接受高强度术前阻力训练计划.比较两组患者干预前﹑术前﹑术后1个月﹑3个月的疼痛视觉模拟(VAS)评分﹑西安大略麦马斯特大学骨关节炎指数量表(WOMAC)评分﹑关节活动范围(ROM)中屈曲和伸展值与健康状况调查简表中生理功能10项(PF-10)评分.结果术前﹑术后1个月﹑3个月,观察组VAS评分分别为(4.22±1.15)分﹑(2.71±0.84)分﹑(1.46±0.53)分,均低于对照组的(5.87±1.60)分﹑(4.50±1.23)分﹑(3.03±0.75)分,组间差异有统计学意义(P<0.05).术前﹑术后3个月,观察组WOMAC评分均低于对照组,PF-10评分均高于对照组,ROM-伸展值均低于对照组,ROM-屈曲值均高于对照组,组间差异有统计学意义(P<0.05).结论高强度术前阻力训练能明显减轻OA患者膝关节置换术后早期疼痛程度,促进关节功能恢复,同时提高患者生理功能,促进患者膝关节置换术后的身体机能恢复.
Objective To study the effect of high-intensity preoperative training on early pain and functional recovery after total knee arthroplasty.Methods 76 patients with osteoarthritis(OA)who underwent knee replacement in the Affiliated Hospital of Chifeng University from January 2021 to April 2023 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 38 cases in each group.All patients underwent knee arthroplasty,with patients in the control group receiving routine preoperative management and patients in the observation group receiving a high-intensity preoperative resistance training program.The Visual Analogue Scale(VAS)score,Western Ontario McMaster University Osteoarthritis Index scale(WOMAC)score,range of motion(ROM)flexion and extension value and physiological function 10 item(PF-10)score of the two groups of patients were compared before intervention,before operation,1 month and 3 months after operation.Results Before operation,1 month and 3 months after operation,the VAS scores of the observation group were(4.22±1.15)points,(2.71±0.84)points and(1.46±0.53)points,respectively,which were lower than(5.87±1.60)points,(4.50±1.23)points and(3.03±0.75)points of the control group.There were significant differences between groups(P<0.05).Before operation and 3 months after operation,the WOMAC score of the observation group was lower than that of the control group,the PF-10 score was higher than that of the control group,the ROM-extension value was lower than that of the control group,and the ROM-flexion value was higher than that of the control group,the differences between the two groups were statistically significant(P<0.05).Conclusion High-intensity preoperative resistance training can significantly reduce the early pain of OA patients after knee arthroplasty,promote the recovery of joint function,improve the physiological function of patients,and promote the recovery of physical function after knee arthroplasty.
